Assist families of infants and toddlers with disabilities in accessing health and developmental services and make referrals to providers for needed services; Provide targeted case management by coordinating, facilitating, and monitoring the provision of early intervention and other services under Part C of IDEA, including evaluations and assessments; Facilitate and participate in teams to develop, review, and evaluate Individual Family Service Plans (IFSP) and ensure appropriate services are being provided; Inform families of their rights and procedures for safeguarding their rights and maintain early intervention records for accuracy and completeness. What you’ll need to be Successful Employees will need to have a commitment to serving diverse infants and toddlers with special needs and their families, have good communication skills, be an effective team player, have knowledge of child development and family-centered practices, ability to develop creative solutions to assist families, have good organization skills, and be able to prioritize work to meet deadlines. They must be able to learn and document in the case management system, as trained by
