About the Position
Provide services to children birth to three receiving services under Early Intervention. This position requires meetings, assessments, TCM billing, interviews, provide guidance to families, provide I.F.S.P., for all families as well as Providers contracted with Early Intervention, Child find activities, under the supervision provided by the Program Coordinator, travel, registering and exiting patients, data entry and other duties as assigned.

  • Assist families of infants and toddlers with disabilities in accessing health and developmental services and make referrals to providers for needed services;
  • Provide targeted case management by coordinating, facilitating, and monitoring the provision of early intervention and other services under Part C of IDEA, including evaluations and assessments;
  • Facilitate and participate in teams to develop, review, and evaluate Individual Family Service Plans (IFSP) and ensure appropriate services are being provided;
  • Inform families of their rights and procedures for safeguarding their rights and maintain early intervention records for accuracy and completeness;
  • Assist families in achieving their goals, including understanding their child’s strengths, abilities, and special needs and how to help their child develop and learn;
  • Facilitate the development of a transition plan to preschool, school, or other appropriate services; and
  • Conduct outreach into the community to ensure all infants and toddlers with disabilities are identified and participate with community stakeholders to enhance the early intervention system.

Successful employees will need to have a commitment to serving diverse infants and toddlers with special needs and their families, have good communication skills, be an effective team player, have knowledge of child development and family-centered practices, ability to develop creative solutions to assist families, have good organization skills, and be able to prioritize work to meet deadlines.
Preferred Qualifications
Typically requires a Bachelor’s Degree and 2-4 years of experience.

Agency preferred qualifications:
  • Bachelor’s Degree from an accredited four-year college or university in Early Intervention/Early Childhood Special Education, Early Childhood Education, Child Development, Special Education, or Social Work
  • At least two (2) years of experience in service coordination for children with disabilities between birth to eighteen (18) years of age or service provision to children under six (6) years of age
