Social Work :
The Non-VA Care Social Work Coordinor applies social work interventions with a high degree of autonomy, exercising independent judgment, creive thinking and initiive in carrying out social work functions and in applying social work values and ethics. The incumbent provides advance level medical, behavioral, and crisis intervention services to Veterans pre-screened by the Transfer Team or referred by colleagues.
Functions:
The incumbent provides medical and behavioral social work services for Veterans who are identified and referred by the CITC transfer teams. The incumbent screens referred pients for potential psychosocial considerions, needs or barriers, then provides options to OSH Case Manager, Veterans, caregivers, assisting them with accessing services if case management is necessarily. The incumbent collabores with Social Work, rehab services, contract programs, VA long term care coordinors, Primary Care, transfer coordinors, and Nursing Service to facilite acute rehabilition admission, transfer to a skilled nursing facility, initiion or resumption of skilled or nonskilled homemaker/home health services, respite, and/or referral to other VA and/or community programs. The incumbent applies case management principles to their pient populion to ensure approprie utilizion of resources and level of care. Psychosocial Screening and Assessment:

The incumbent will assess for functional and cognitive stus, syndromes, illnesses, and impairments of vulnerable and elderly Veterans.
Psychosocial Trement and Intervention:
The incumbent works closely with Veterans receiving CITC to identify and support their preferences in care and personal trement goals, and offers resources and support tailored to the uniquely expressed needs and desires of the Veteran The incumbent serves as the key link for educion and psychosocial support for Veterans, families, caregivers or/and outside hospital community partners The incumbent participes in the creion of the Veteran’s trement and/or admission plan, with special tention to the psychosocial elements and interventions of the trement and/or admission plan.
Specialized Case Management and Care Coordinion:
The incumbent serves as a champion for Care Coordinion and Integred Case Management. The incumbent participes in the training of Social Workers, Peer Support Specialists, volunteers, and students; participes as required and requested in oriention, training and staff development programs involving Social Workers across the Health Care System.

The program coordinor may be the sole practitioner in this specialty the facility and typically provide direct pient care services in the program area. The program coordinor oversees the daily operion of the program, develop policies and procedures for program operion, and prepare reports and stistics for facility, VISN, and nional use. They may be responsible for the program’s budget, developing and monitoring staff compliance with practice, standards and guidelines on documention, workload, da entry, ethical practice and service delivery. The program coordinor provides analysis and evaluion of clinical program da and computerized programs to identify system-wide trends and needs to enhance the quality of service.
