Healthy Opportunities Pilot Case Manager Destiny Total Healthcare 407 East Fremont Street, Burgaw, NC 28425 Job Overview As a Human Services Organization (HSO), Destiny Total Healthcare HOP, will provide Transportation, Housing Move -In Support, Essential Utilities Set Up, One Time Payment for Security Deposit and First Month Rent, Holistic and IPV Sensitive Services Support in Pender and Onslow County, NC. The case manager is responsible to ensure the HSO is delivering services in compliance with policies. This position is full time. We are seeking a dedicated and compassionate Case Manager to join our team. The ideal candidate will play a vital role in supporting individuals through various challenges, including behavioral health issues, crisis situations, and workforce development. This position requires a strong commitment to helping clients achieve their goals and improve their overall quality of life. Duties Work closely with the Program Director to deliver Healthy Opportunities Program services per the HSO Manual Participate in readiness reviews for program start-up and continuation Use Unite Us for Pilot Service referrals, invoices, and reporting when needed. Report service delivery and related outcomes Invoice for delivered authorized pilot services through NCCare 360 (training will be provided) Support identification and enrollment of pilot-eligible individuals Participate in pilot-related training and technical assistance efforts. Participate in pilot quality improvement, program evaluation, and program integrity Participate in pilot-related reporting and evaluation.

Experience in workforce development is highly desirable. Knowledge of behavioral health practices and principles. Familiarity with motivational interviewing techniques. Understanding of Section 8 housing programs and related resources. Strong interpersonal skills with the ability to build rapport with diverse populations. Excellent organizational skills and attention to detail are essential for maintaining accurate records. $20.00 – $22.00 per hour Expected hours: 40 per week
