St. Agnes is looking fora Stroke Coordinator/Inpatient Rehab Coordinator to join ourteam!

Schedule:

36 hours/ days.

Responsibilities:

The role is responsible for Joint Commissioncertification of our Stroke Program, running our stroke data andmeetings and also rehab data. This person will support the IPRsocial worker and will assess prospective patients for theirability to meet the IPR criteria for admission to that unit. Community outreach regarding stroke and prevention. No direct reports to this role.

Providescoordination of the activities and flow of work for the assignedclinical program.

Job Responsibilities and Requirements:
PRIMARYRESPONSIBILITIES

Facilitates the development, implementation and evaluation ofprogram services and initiatives. Plans, implements, evaluates, and coordinates appropriatepatient care. Maintains program documentation and review to supportregulatory and compliance regulations. Assists with determining appropriate training for the staff anddeveloping the learning program. Collaborates with advanced practice providers to ensure qualityoutcomes. Collaborates with others to assist with problem identificationand resolution, cost containment issues, implementation of newservices, and systems/performance measures. May participate in injury prevention initiatives and/oractivities. Works in a constant state of alertness and safe manner. Performs other duties as assigned.

EDUCATION

Bachelor’s degree in Nursing or equivalent combination ofeducation and experience

EXPERIENCE

Three years’ registered nurse experience

PHYSICAL REQUIREMENTS

Frequent lifting/carrying and pushing/pulling objects weighing0-25 lbs. Frequent sitting, standing, walking, reaching and repetitivefoot/leg and hand/arm movements. Frequent use of vision and depth perception for distances near(20 inches or less) and far (20 feet or more) and to identify anddistinguish colors. Frequent use of hearing and speech to share information throughoral communication. Ability to hear alarms, malfunctioningmachinery, etc. Frequent keyboard use/data entry. Occasional bending, stooping, kneeling, squatting, twisting andgripping. Occasional lifting/carrying and pushing/pulling objectsweighing 25-50 lbs. Rare climbing.

REQUIRED PROFESSIONAL LICENSE AND/ORCERTIFICATIONS
  • Resuscitation Certification Requirement:

    Basic LifeSupport Healthcare Provider (BLS HCP) required within 90 day ofemployment. Other resuscitation certifications may be requireddependent on department.
