Human Services Provider Program Coordinator I Position Available In Wayne, North Carolina

Come join our TEAM! The primary purpose of this position is to serve as a liaison between families, legal representatives, and/or significant others of residents served by O’Berry Neuro-Medical Treatment Center. To develop one to one relationships with residents and their families through visits, activities, correspondences, and phone calls. To counsel families related to resident’s physical limitations and issues associated with death and dying. To notify families of any emergency issues related to residents. To assist in new applicant screenings and admission process. Assist the Transition Coordinator with admissions as needed. To participate as part of the interdisciplinary team in resident assessment, care plan process, care plan and interim care plan meetings. To write quarterly and annual progress notes for each resident.

O’Berry Neuro-Medical Treatment Center is a specialized skilled nursing facility focused on maintaining the quality of life and dignity for adults with neurocognitive disorders, with coexisting complex medical and behavioral needs, whose level of support extends beyond which can be provided by family, or in a community living setting.” Thorough knowledge of the concepts, practices, and techniques used in the field of neurocognitive disorders to include program development and coordination, facilitation of service delivery Knowledge of Certified Medicaid Services (CMS) or OBRA regulations and methods of ensuring compliance with such rules and standards, program facilitation, staff training and monitoring Demonstrated ability to analyze data and determine trends and patterns Demonstrated ability to write person centered habilitation plans or care plans and reports Demonstrated ability to develop cooperative working relationships with a variety of people, who provide services to individuals with neurocognitive disorders in a calm and professional manner Demonstrated ability to work as part of an interdisciplinary team in order to deliver necessary services Some state job postings say you can qualify by an “equivalent combination of education and experience”. Bachelor’s degree in a human services or related field from an appropriately accredited institution, and one year of experience providing and/or developing person-centered care plans for individuals with intellectual disabilities/co-occurring disorders OR an equivalent combination of education and experience. Advanced degrees in human services may be substituted to meet educational requirements if an applicant does not possess a Bachelor s degree in human services or related field.
