Job Description
Come join our TEAM! The primary purpose of this position is to serve as a liaison between families, legal representatives, and/or significant others of residents served by O’Berry Neuro-Medical Treatment Center. To develop one to one relationships with residents and their families through visits, activities, correspondences, and phone calls. To counsel families related to resident’s physical limitations and issues associated with death and dying. To notify families of any emergency issues related to residents. To assist in new applicant screenings and admission process. Assist the Transition Coordinator with admissions as needed. To participate as part of the interdisciplinary team in resident assessment, care plan process, care plan and interim care plan meetings. To write quarterly and annual progress notes for each resident.

Bachelor’s degree in a human services or related field from an appropriately accredited institution, and one year of experience providing and/or developing person-centered care plans for individuals with intellectual disabilities/co-occurring disorders OR an equivalent combination of education and experience. Advanced degrees in human services may be substituted to meet educational requirements if an applicant does not possess a Bachelor s degree in human services or related field.
