Full Job Description Description:

Greater Nashua Mental Health is looking for a CTI Team Leader to join our team! The Team Leader oversees, supports, trains, and manages the Critical Time Intervention Team (CTI). Ensures that the team is following the CTI fidelity model in accordance with the state contract. Communicates with the State Hospital and DRF hospitals to support referrals. Evaluates and assesses individuals who are hospitalized for CTI services. Works to connect CTI clients to longstanding GNMHC services. Provides services to a small caseload of CTI clients.

Duties & Responsibilities:

Supervises, supports, trains, and leads CTI Specialists in delivering services in alignment with the CTI model Develops, aggregates, and analyzes program data and provides on demand reporting to GNHMH Leadership and to the state Monitors the CTI contract and delivery of CTI services to be congruent with the CTI fidelity model Attends state level meetings related to the CTI program and provides guidance and feedback to the ES Program Manager and ES Director on updates to the program Provides individual and group supervision to CTI staff, weekly and in accordance with the CTI contract Engages New Hampshire Hospital and Designated Receiving facilities in CTI client recruitment and provides CTI screenings and assessments to eligible clients Promotes and provides education about the CTI program and services to GNMH programs and community stakeholders Maintains and provides services to a small caseload of CTI clients (10-15) Works with GNMH teams to transition clients in the CTI program to longstanding GNMH services Completes all documentation in a timely manner following GNMH protocols Performs other duties or special projects as required or as assigned

Requirements:
Qualifications:

Bachelor’s Degree in Mental Health or related field 3 to 5 years related work experience Ability to lead, train, and supervise a team of 3-4 staff to deliver mental health services Ability to problem solve with staff and clients to ensure clients’ basic needs are met Ability to develop and analyze program data and to complete data reporting as needed Ability to work effectively with individuals experiencing social and emotional problems Knowledge of CTI fidelity model and ability to prioritize delivery of mental health services in alignment with fidelity Strong networking and relationship building skills Strong collaboration skills, and an ability to work effectively both independently and as part of a team Comfortable in a fast-paced environment with frequent deadlines; high energy, self-motivated and proactive Excellent communication and interpersonal skills Ability to multitask, and be flexible with regard to workload and assignments Ability to problem solve and exercise judgment Knowledge of Microsoft Office products (Email, Word, EXCEL) Must possess a valid driver’s license and automobile with liability insurance in the amount required by agency policy
