Brain Injury Association of New Hampshire – Associate Executive Director Direct Services Work with individuals and families who are living with disabilities under the

CFI/ABD/DD

waiver systems and provide coverage as needed Maintain a small, personal caseload of individuals receiving case management or service coordination under the

CFI/ABD/DD

waiver systems Provide short-term coverage for other service coordinators or case managers caseloads, including travel and in-home visits, as needed Work with individuals at risk of abuse or exploitation referred to BIANH by local Adult Protective Services office Meet with individuals and families in their homes to develop a plan of care Gather pertinent information and records pertaining to the individual Review and organize information gathered from various sources Consult with individuals and/or families to identify concerns and applicable services Conduct ongoing assessment and update planning document as needed Coordinate the delivery of services to meet identified goals and objectives Advocate for equal access to services to maximize individual functioning and opportunities for inclusion Consult with individual/family to ensure that services being received are appropriate, timely and meeting identified goals and objectives Consult with providers to ensure that the services provided are appropriate, timely and meet identified goals and objectives Guide individuals/family in resolving difficulties with agencies, access to services and/or service delivery Supports self-advocacy efforts Maintains information and records in accordance with established policies and procedures Back up support to office phones, information and resources program Assist regarding grants/contracts Represent BIANH on assigned committees Indirect Services Serve as a consultant on brain injury issues for agencies and providers to ensure equal access to services and equitable treatment for all individuals Provide training and outreach to community agencies Maintain confidentiality and standards of ethical practice Qualifications Bachelor’s Degree in Counseling, Social Work, Psychology, Sociology or related field Understanding of disability issues Excellent communication skills necessary, both written and verbal Computer skills Ability to work independently Valid driver’s license, reliable transportation, and adequate vehicle insurance coverage Must past background check, which includes, criminal and
