PILOT Program Coordinator 4.2 4.2 out of 5 stars 207 Locust Street, Dover, NH 03820 Position Description Dover Children’s Home is seeking a compassionate, highly skilled PILOT Program Coordinator to join our dedicated team. This individual will work with young adults ages 18- 21, their families, support networks, and community-based agencies to provide case management within the PILOT Program, oversee transition planning activities, and support young adults in healing from trauma. The PILOT Program Coordinator will be responsible for supporting advocacy, coordination of independent living skills building, transition planning, operational support, and maintaining strong administrative and financial practices. This position is under the direction of the PILOT Program Manager. SUPERVISION
Provide daily support and observation of graduate level interns within the PILOT Program. Prepare for and attend weekly supervisions with the PILOT Program Manager as the supervisor for this role.
TREATMENT SERVICES
Treat young adults, families, colleagues, stakeholders, and community members with respect, empathy, and dignity at all times. Provide direct oversight and supervision of young adults in care, while supporting case management and transition coordination. Utilize safety plans, provide crisis intervention, and coordinate care for young adults, as needed. Collaborate with DCH’s coordination team to provide comprehensive and holistic treatment services for all youth and young adults in care by developing and implementing individualized independent living plans based on evidence-based practices and a trauma-informed approach. Advocate for young adult’s identified transition goals and support the young adult’s goal progress toward independent living plan in collaboration with the team. To honor young-adult’s voice and choice, work collaboratively with those served to schedule and coordinate employment, driver’s education, medical appointments, educational needs, and independent living skills building, while providing communication to the young adult’s natural supports, mentors, state workers, and/or additional team stakeholders. Develop, implement, and engage young adults with intentional and ongoing groups, aimed to provide education and develop competencies in adult living. Support each young adult’s independence by providing guidance, learning opportunities, and access to resources regarding finances, activities of daily living, community providers, and personal documentation. Attend all young adult-related meetings, including Independent Living Plan meetings, RENEW meetings, IEP meetings, etc., as appropriate. Foster relationships with community-based agencies to build connections and increase services for young adults. Conduct comprehensive independent living assessments with young adults during their intake, throughout their program stay, and to determine and support their permanency goals upon discharge according to all state and regulatory timelines. Complete thorough documentation to include case notes, daily charts, incident reports, monthly reports, independent living plans, required assessments and/or discharge documentation within the identified timeframes. Participate in the intake process by providing tours, completing intake assessments, and orienting new residents to the Dover Children’s Home. Ensure that all services and documentation are accurate, up-to-date, and in compliance with accreditation standards, ethical practices, and all other regulatory bodies. Uphold the principles of Trust-Based Relational Intervention (TBRI) in all interactions with young adults ensuring fidelity to the model of care. Facilitate Resiliency, Empowerment, Natural supports, Education and Work (R.E.N.E.W.) model with young adults, their teams, and staff to at least 80% fidelity of the program, in collaboration with DCH’s

team. Coordinate volunteer participation within PILOT Program to build natural supports and increase opportunities for young adults in collaboration with the DCH Volunteer Team and PILOT Program Manager. Oversee self-administration of medication for young adults as outlined by DCH’s policy and procedure manual and medication self-administration process and participate in Level 1 medication reviews. Provide up to 6-months of after-care services to discharged young adults and coordinate monthly resource night activities for any/all past DCH residents in collaboration with DCH’s aftercare team, as assigned.
OPERATIONAL
Ensure timely and effective communication with all stakeholders (family members and supports, case workers, guardians, if applicable, community-based providers, etc.) regarding transition plans, progress, and concerns. Maintain electronic records, treatment documentation, and other young adult needs in compliance with regulatory standards, confidentiality laws, and Dover Children’s Home policies. Complete all training requirements thoroughly for role, agency, and license (if applicable) within the identified timeframes. Participate in the on-call rotation. Maintain responsibility for the physical safety and cleanliness of the organization, including personal offices/office spaces and shared spaces in the programs. • Promptly report any household safety concerns to the Facilities Coordinator and Director Team.
ADMINISTRATIVE
Follow organizational protocols for accepting and documenting donations, ensuring proper communication with relevant team members. Represent the organization with professionalism and gratitude toward donors, emphasizing the importance of their contributions.
FINANCIAL
Make fiscally responsible decisions that align with organizational policies, ensuring proper oversight and minimizing financial burdens on the organization. Adhere to all financial policies and procedures to maintain accountability and transparency. Communicate yearly budgetary needs and financial requests to the PILOT Program Manager as requested. Ensure timesheets are up-to-date, accurate, and overtime is managed in a fiscally responsible way.
