Position Summary
The Program Coordinator will be responsible for continued growth and success of the Palliative Care and Hospice Program while planning, coordinating, implementing, and evaluating the administrative operations and clinical activities on a twenty-four hour basis; will provide coordinated services that are seamless, and promote an integrated system of care that is high value, cost effective, and customer focused.
Core Duties and responsibilities:
- Assists director in continuous operation of the program
- Develop and direct oversight of patient programs including outreach to physicians and providers for education and referral into program (example: Healing Touch, Aromatherapy, Palliative Care)
- Facilitates delivery of patient care according to departmental and unit specific policies
- Participates in the selection, training, scheduling and/or development of human resources.
- Facilitates patient flow
- Participates in daily unit rounds as able
- Collaborates with team members regarding patient status and ongoing planning needs
- Serves as a professional practice resource for patients and families in need of specialized care.
- Identifies needs, plans and implements educational programs within specialty area for staff, patients/families and or community groups.
- Serves in consultant role to staff, management, health care professionals, and or lay people within the hospital and or community.
- Develops a scientific basis for practice through informal and formal research activities related to area of practice.
- Provides, manages and evaluates clinical patient care.
- Assesses learning needs of clinical staff and develops a plan to provide appropriate educational experiences.
Requirements:
- Valid NJ APN or PA license
- 5 years of Palliative Care experience
- IV certification
- National certification
Preferred Requirements:
- Grant Writing Experience
- Certifications in Hospice and Palliative Nursing
Salary Range – $80,000 – $90,000 annually
