Summary As a Case Manager Specialist Supervisor at The Arc of Cape May County, you will play a vital role in supporting individuals with intellectual and developmental disabilities. This position involves supporting the teams providing services and supports to individuals with intellectual or developmental disabilities in planning, organizing, and coordinating, supports and services to ensure they are receiving the highest quality care. This includes monitoring electronic health records, medical appointments, meetings, critical logs, and other relevant documentation to ensure information is up to date and accurate. This role provides guidance and support to the agency and residential teams to ensure quality standards are consistently met and healthcare and social services needs are satisfied routinely and as needed. This position directly supervises the Case Management Specialists. Responsibilities Organizes and coordinates health care appointments and provides service delivery of the same as needed. Serves as primary point of contact for scheduling of annual medical and dental services including routine specialty care. Provides reminders and follow up communication and documentation to staff in residential and day program settings as needed for coordination of transportation and setting calendars for appointments to provide most efficient use of resources available. Completes comprehensive and detailed review of NJISPs to ensure accuracy, consistency, and accountability in coordination of health care and social services supports. Communicates with support coordinators and agency staff regarding updates and revisions needed to NJISPs, including the completion of revision worksheets and supporting completion of all NJISP planning activities. Schedules planning team meetings for annual reviews and as needed. Coordination and review of documentation needed, including assessments and other available records, prior to admission, upon transfer, during stay, and readmission of person supported and upon discharge to ensure continuity of care during their stay at The Arc. Reviews electronic health records, forms, correspondence, service plans, medications, & communications (Tlogs). Assists with monitoring medication administration records (eMAR), especially as it relates to the pass along of information to the medical providers, and monthly reconciliation of medications. Serve as the primary contact for pharmacy providers to ensure medications for individuals supported and necessary documentation, including configuration within the eMAR (Therap) system. Follow-up with external agencies including health care professionals, psychiatrists, medical specialists, dental providers, physical and occupational therapy, day service programs, and other external partners as needed to ensure necessary care, follow up care, and continuity of services. Attends training/continuing education and continues to remain knowledgeable about current best practices in services to support people with IDD. Maintains a customer service focus that is proactive and preventative and responsive to concerns or complaints as needed. Completion of all other duties that are needed to support the service recipients, administrative & direct support team(s), and residential & day programs. Requirements Bachelor’s Degree (BSW, Psychology, Health care related, or other related degree) preferred; in the absence of degree, must actively be currently enrolled towards completion. Five years in social service, health care field; experience working within NJ ID/D services and familiarity with services and supports within the Cape May County and surrounding areas preferred. Prior case management and case coordination and/or health care experience strongly encouraged. Working knowledge of electronic health records (Therap experience preferred); ability to engage others in collaborative manner; willingness to adapt to changing circumstances including duties assigned based upon best interests of the persons and the agency; high energy and enthusiasm with positive thinking and goal directed towards positive outcomes.
