Summary We are seeking compassionate and skilled Case Management Specialists to join our team at The Arc of Cape May County. In this role, you will support the case management department specifically in the provision of services and supports to individuals with intellectual or developmental disabilities for planning, organizing, coordinating, and facilitating health care related activities. This includes monitoring electronic health records, medical appointments, critical logs, and other relevant documentation to ensure information is up to date and accurate. This role provides guidance and support to the agency and planning teams to ensure quality standards are consistently met and healthcare and social services needs are satisfied routinely and as needed. Responsibilities 1. Organizes and coordinates health care appointments and provides service delivery of the same as needed. 2. Serves as primary point of contact for scheduling of annual medical and dental services including routine specialty care. 3. Provides reminders and follow up communication and documentation to staff in residential and day program settings as needed for coordination of transportation and setting calendars for appointments to provide most efficient use of resources available. 4. Comprehensive and detailed review of NJISPs to ensure accuracy, consistency, and accountability in coordination of health care and social services supports. Communicates with supervisor regarding updates and revisions needed to NJISPs, completes revision worksheets and supporting completion of NJISP planning activities as directed. 5. Coordination and review of documentation needed, including assessments and other available records, prior to admission, transfer, re-admission of person supported and upon discharge to ensure continuity of care during their stay at The Arc. 6. Reviews electronic health records, forms, correspondence, service plans, medications, & communications (T-logs). 7. Assists with monitoring medication administration records (eMAR), especially as it relates to the pass along of information to the medical providers, and monthly reconciliation of medications. 8. Follow-up with external agencies including health care professionals, psychiatrists, medical specialists, dental providers, physical and occupational therapy, day service programs, and other external partners as needed to ensure necessary care, follow up care, and continuity of services. 9. Attends training/continuing education and continues to remain knowledgeable about current best practices in services to support people with IDD. 10. Maintains a customer service focus that is proactive and preventative and responsive to concerns or complaints as needed. 11. Completion of all other duties that are needed to support the service recipients, administrative & direct support team(s), and residential & day programs. Financial 1. Assist in meeting the needs of clients through the use of individuals insurance and provides receipts for any approved expenses incurred by the individual or the agency during the course of facilitating the appointment(s). Documentation Maintains timely, detailed documentation to ensure adequate funding levels are met and compliance with state agencies is captured while simultaneously recording individual progress which will ensure continuity of support to clients.

Specifically:

1. Completes incident reports and other reports as required by state licensure and agency procedures. Reports significant incidents and concerns to appropriate supervisory staff and on-call staff according to agency procedures and regulatory requirements. 2. Completes daily documentation for completion of medication administration when applicable and health- related activities as required by state licensure and agency procedures. Documentation includes thorough and complete details. 3. Completes critical log (Tlog) documentation regarding the appointment, routine schedule or unplanned, and relevant outcomes of the appointment. Direct Support Services Provide personalized, one-on-one daily support to clients to attend to their physical and emotional needs in a safe and nurturing environment that promotes self-determination, respect, fairness and integrity.

Specifically:

1. Ensures that individuals receive meaningful services in community settings that reflect the core values of People First, Equity, Community, Self- determination and Diversity. Act as an advocate when interacting with the community. 2. Conscientiously completes medication administration and health- related activities as required and trained according to the individual’s service plan, and according to the requirements of agency procedures and state licensure. Maintains up-to-date knowledge of state and agency procedures and consults policy manuals when needed. 3. Performs first aid and other emergency procedures for individuals served according to the immediate needs and practices as trained in certification courses. 4. Provides monitoring, supervision, assistance, skill and goal development, along with behavior support and intervention to individuals served to ensure safety, health and training needs are met according to the individual’s service plans. Supports self-determination by engaging clients in decisions, respecting their autonomy and encouraging growth and independence. 5. Provides assistance with basic personal care and grooming, including bathing, dental and hair care, toileting, dressing and clothing/laundry care to individuals as assigned, and according to the needs and requirements of the service plans. Demonstrates respect for the human dignity of individuals by proactively attending to their needs without being asked. 6. Transports individuals served to community sites for medical appointments, including supervision at these activities. Acts as their eyes and ears in the community and encourage inclusion. Requirements 1. Must be at least 18 years old and have a High School Diploma or equivalent, Associate’s Degree or 60 credit hours towards related degree; in absence of degree, must be currently actively enrolled toward completion. 2. Completion of pre-service classes and training as required by state licensure and agency procedures, including coursework and certification in First Aid, CPR, crisis management, behavior supports and other areas. 3. Current driver’s license valid in the state of New Jersey and driving record acceptable to insurance and regulatory requirements. 4. Must possess basic skills for following written and verbal instruction, and for communicating client and job related issues verbally and in writing, including the agency electronic data base systems. Must have the ability to communicate with individuals with whom they are working. Here’s a brief summary of the benefits offered: Health, dental, and vision benefits are available on the first day of the month after 60 days of full-time employment. The agency offers a two different plans with Aetna, one of them being a HSA plan. Employer paid group term life insurance is available to you after five months of full-time employment. You can enroll in the Principal Retirement Plan immediately, and be eligible for up to 3% matching contributions after a year of employment. We offer personal time-off, vacation time, and sick time. Sick time is accrued one hour for every thirty hours worked and can be used beginning 120 days after start of employment. You’ll receive one personal day after 120 days of employment, and then two more days upon anniversary date, and accrue vacation time of 6.67 hours per month starting after 120 days of employment.
