Summary The Case Coordinator – Housing is responsible for tracking housing-related case management services and outcomes. Responsibilities Organize and input program data into HMIS, agency electronic health record, and other data management systems as needed. Track and analyze program performance metrics, identifying trends and areas for improvement. Generate regular reports for internal review, funders, and other stakeholders. Maintain accurate, up-to-date records of participant information, services provided, and outcomes. Ensure data quality by performing regular audits and resolving discrepancies. Train staff on data entry protocols and best practices to ensure consistency across the program. Recommend and implement improvements to data collection and reporting processes. Work closely with the Housing Director and other staff to align data tracking with program goals and outcomes. Provide insights to support quality improvement activities. Assist in developing and maintaining documentation of data management policies and procedures. Assist with other tasks as assigned.

Required Education and Experience:

High school diploma or GED. A minimum of one (1) years’ experience in data entry and management.

Specialized Skills, Abilities and Knowledge:

Experience working with database systems such as HMIS or similar platforms is highly desirable. Proficiency in Microsoft Excel and other data analysis tools. Strong attention to detail and commitment to data accuracy and integrity. Experience interacting with electronic health records preferred. Excellent organizational skills with the ability to manage multiple priorities. Strong interpersonal and communication skills with the ability to collaborate with team members. Knowledge of substance use disorder treatment, mental health treatment, recovery, and/or social service programs preferred.
