SERVICE AREA – MERCER, MIDDLESEX, SOMERSET, BURLINGTON, AND CAMDEN COUNTIES
The ESC Support Coordinator is an organized, motivated person who works well in both independent and team settings. The Support Coordinator manages services for 35 individuals with intellectual/developmental disabilities.
Support Coordinator Responsibilities:
Develop Individual Service Plan (ISP) and Person-Centered Planning Tool (PCPT) for individuals enrolled in DDD/Medicaid services. Connect the individual with services and supports. Develop a thorough understanding of the services funded by DDD and other state and community resources, to ensure that individuals/families are offered informed choices or services and providers. Adhere to deadlines – submit new plans within 30 days of enrollment, conduct plan renewal meetings at least 60 days prior to renewal. Submit plan renewals to Supervisor at least 30 days before plan renewal dates. Maintain monthly contact, or more frequently as needed. Complete required documentation via the Monthly Monitoring Tool. Monthly contact via telephone, face-to-face or home visits that include review of the ISP and documentation on the Monthly Monitoring Tool. Complete at least 12 hours per year of continuing education training. Attend monthly staff meetings in Hamilton, NJ. Office attendance once a week. Required Knowledge/Skills/Attributes Strong and effective spoken and written communication skills. Respect and appreciation for people with disabilities, their families, other organizations, agencies, volunteers, and consumers. Experience and proficiency in using Office 365 products – Word, Outlook, Teams, and Excel Ability to work independently with minimal direction with support of ESC team when guidance is needed. Represent ESC in a professional manner. Education and Experience Bachelor’s degree in any field and 1 year of experience working with adult (18 or older) individuals with developmental disabilities. The experience must be the equivalent of a year of full-time documented experience working with adults. The experience can include paid employment, volunteer experience, and/or being a family caregiver of an adult with a developmental disability. State, Federal Criminal Background checks and Central Registry Check at time of hire. Successfully complete trainings required by the Division of Developmental Disabilities before rendering services. College of Direct Support trainings. Working Conditions Standard work week is 9:00 AM – 5:00 PM – yet requires flexibility to meet the requests for consumers/families meeting after standard working hours. On-Sie – 1 day per week in Hamilton NJ office Requires in-home visits and attendance at community-based meetings, therefore Support Coordinator must have reliable transportation.
