DWDC is a mission-driven organization committed to addressing health-related social needs through culturally responsive, community-based care. We empower women and their families by connecting them to essential services that promote health, stability, and dignity. Position Summary We are seeking a dynamic and experienced Program Supervisor to lead a multidisciplinary team of Screener Navigators, Care Managers, and a Nutritionist. This role is pivotal in ensuring high-quality service delivery, operational excellence, and alignment with DWDC’s mission to build healthier communities. The ideal candidate will have a strong background in program and relationship management. This role requires a compassionate leader who can effectively supervise staff, manage budgets, and ensure the delivery of high-quality services. Key Responsibilities Provide day-to-day supervision, coaching, and performance management for a team of social care professionals Oversee workflows and ensure timely, client-centered delivery of services including screenings, care management, nutrition support, and referrals Monitor caseloads, documentation, and outcomes to ensure compliance with program standards and funder requirements Facilitate team meetings, case conferences, and ongoing training to support staff development and collaboration Serve as a liaison between frontline staff and leadership to communicate needs, challenges, and successes Develop and implement program policies and procedures that align with organizational goals. Conduct regular assessments of program effectiveness and make necessary adjustments to improve outcomes. Analyze program data and contribute to quality improvement initiatives Ensure compliance with all regulatory requirements Provide training and support to staff on best practices in social work and program development. Foster a culture of accountability, empathy, and continuous learning Qualifications Bachelor’s degree in Social Work, Public Health, Health Administration, or related field (Master’s preferred) Minimum 3-5 years of supervisory experience in a healthcare, social services, or community-based setting Strong leadership, communication, and organizational skills Experience working with diverse populations and a commitment to health equity Bilingual skills (especially Spanish) are a plus
