Full Job Description SUMMARY:
Responsible for oversight of supervisory, programmatic and instructional aspects of the site-based Group Day Habilitation program.
ESSENTIAL
DUTIES and/or
RESPONSIBILITIES
include the following. Responsible for the organizing, coordinating and implementing of the GDH Day Habilitation Program. Provide direct, day to day supervisory responsibility for the GDH Day Habilitation program staff (Direct Support Professionals). Responsible for completing Core Competencies evaluations for all assigned staff. Oversee the planning, writing, and managing the schedule of daily activities and group assignments for the GDH program areas. Build relationships within the community to promote community integration opportunities for individuals in the program. Assume primary responsibility for monitoring and coordinating the paperwork flow and billing associated with the program. Responsible for ensuring that billing documentation is completed in a timely fashion and review billing against attendance to ensure accuracy. Maintain record of staff attendance (ADP), schedule and approve all GDH staff PTO requests. Monitor staff training and ensure that all staff has the skills and knowledge needed to provide support to assigned individuals. Manage the needs of individuals in the program within the scheduled activities to ensure that individuals are receiving appropriate; person centered, and individualized supports within the group setting of the program. Conduct program observations in site based and community settings to ensure that supports provided are Person Centered and within the specified Day Habilitation Plan for each individual. Initiate any new methods and/or ideas which will lead to more effective training of participants. Collaborate with Behavior Management Coordinator and Registered Nurse as participant needs require. Maintain any records, charts, etc. as directed by the Day Habilitation Program Manager. Assume other responsibilities as specified by the Day Habilitation Program Manager or Associate Executive Director for Programs. EDUCATION and/or
EXPERIENCE
- Associate Degree in Human Service or education related field, or a High School diploma or equivalent and at least 3 years’ experience in a human service agency.
- Experience with developmentally disabled preferred.
- Supervisory experience is expected.
- A clean valid New York State driver license is required.
