Habilitation Coordinator 3.2 3.2 out of 5 stars 33 Denison Parkway West, Corning, NY 14830
Pay:
$20.00 – $22.50 per hour! (depending on related experience)
Nature of Work:
The Habilitation Coordinator will attend Life Plan meetings and write plans for individuals in Community Habilitation and the Employment Services programs and will train staff to implement said plans. They will maintain compliance with all pertinent regulations, policies and procedures, and agency philosophy. They will supervise program staff in the absence of the Program Manager to ensure quality services are provided, including training, scheduling, communication, and service implementation. They will assign a mentor, and monitor completion of staff training and paperwork. They will work collaboratively with the Circle of Support and all other providers to assure person centered services are implemented to individual’s satisfaction. Community Habilitation Minimum Qualifications:
Associate’s degree from an accredited program in the human services or related field, three (3) years’ work experience with developmentally disabled individuals and one (1) year supervisory experience preferred OR a High School diploma, five (5) years’ work experience with developmentally disabled individuals and two (2) years supervisory experience preferred. A valid driver’s license passing insurability guidelines. Must meet the requirements related to Criminal History Record checks that became effective April 1, 2005, which requires prospective employees to be fingerprinted if they have regular and substantial unsupervised or unrestricted physical contact with people receiving services in our programs regulated by OPWDD.
