JOB SUMMARY

Under the direction of the Senior Planning Specialist, this position ensures person-centered-services are provided. This position will ensure all service delivery plans under the program’s auspices, are written in a cohesive manner, ensuring services are aligned with one another to reflect the agency’s person-centered philosophy and approach to service planning and delivery. This position will ensure communication and information distribution within each person’s circle of support. This includes the person, their natural supports, Care Coordinators/Care Managers, agency services, and external services. The Planning Specialist will also facilitate intake for the traditional services offered by the organization. The Planning Specialist, QIDP will perform all QIDP functions per regulation and as outlined in this job description, JOB

FUNCTIONS

Regulatory and Corporate Compliance Ensure conformance with applicable regulations and policies. Ensure all Staff action and other plans are written in accordance with applicable regulatory and billing requirements. Ensure all regulatory and billing deadlines are met. Ensure records, files and reports are kept current and meet regulatory requirements (both in the program of service and the Agency Management Platform). Ensure associates understand and accurately implement plans of service. Ensure adherence to incident management process. Review, monitor, and take corrective action to ensure systems and processes are effective. Develop and implement improvements and corrective actions based on survey results. Ensure Chain of Command is followed when necessary and instructions are followed as stated in policy and procedures. Ensure required documentation is complete, accurate, and legible. Promote the highest quality of support by implementing the Code of Ethics, Core Competencies and Agency I HEART philosophy and practices. Ensures all regulatory QIDP duties are completed. Financial Management Adhere to the program budget as it relates to their position. Remain up to date on all required billing and regulatory elements of services and service plans. Report any billing concerns to supervisor and or Corporate Compliance. Assist in the maximization of billing by ensuring review of all services/plans are completed per Agency and regulatory expectations. When providing services, ensure waiver and other service billing is completed accurately. Person Centered Services Advocate with the person in the development of his/her Life Plan and associated Staff Action Plans. Ensure the person’s Life Plan, assessments, training, and objectives are person directed and reflect the person’s quality of life choices and support systems. Oversee the development and author the person’s Staff Action Plan; ensuring the Personal Outcome Measures, valued outcomes, and Provider Assigned Goals from the Life Plan, accurately reflect the person’s desired outcomes. Ensure the person is the leading driver of the person-centered planning process as well as seeking input from the person’s circle of support. Be an active member of each person’s Interdisciplinary Team (IDT). Facilitate the review and update of Person-Centered tools and assessments. Provide training, feedback, reinforcement, and corrective coaching to associates’ implementation of the Staff Action Plan. Regularly attend program planning team meetings to ensure communication with program/management staff. Assist programs with understanding the Life Plan, Staff Action Plan, and safeguards. Work collaboratively with others to ensure the person’s health and well-being. Work closely with service providers and Care Coordinators/Care Managers to ensure effective communication and that Staff Action Plans are coordinated and consistent with the person’s stated goals and objectives. Assist the Behavior Intervention Specialist with gathering and distributing information with the person and the IDT. Facilitate person centered planning in the intake process. Respect the Person’s right to privacy and confidentiality by appropriate use of written and verbal communication. Provide Direct Support Services to people as outlines in the person’s Life Plan, Staff Action Plan, Individual Plan of Protective Oversight/Safeguards, Support Plans and PONS, ensuring all documentation is completed per Agency/Department expectations. This includes planning and coordinating activities in line with the person(s) stated goals. Maintaining a working knowledge of all above documents. Teach skills as directed by Plans, including the ability to adapt to different learning styles. Reporting and documenting all medical and other concerns to appropriate people. Providing services in line with the NADSP Core Competencies, Code of Ethics, and Agency I HEART philosophy and practices. Observe the implementation of the Staff Action Plan in program and community settings to ensure the Staff Action Plan is implemented and documented, as intended and required. QIDPs are responsible for the following for all people (or a caseload of people) receiving Res Hab services via

Review and sign off on a person’s plan of services every six months. Completing an annual summary assessment of a person’s status using the Life Plan Meeting Summary Form or other appropriate forms. Ensuring that a person’s plan of services is documented appropriately. Verifying that a person participated in the program planning process. Verifying that a provider respects a person’s cultural and language needs. Program/Service Management Distribution of all agency Staff Action Plans, as well as Safeguards, Individual Plan of Protection (IPOP), Plans of Nursing Services (PONS), Behavior Support Plans (BSP), and person-centered tools to appropriate internal and external parties as dictated by Regulation and Agency policy and procedure. Reviews service documentation to ensure the service plan is being implemented as intended. Review monthly summaries to ensure it supports the person’s valued outcomes and services are being implemented as intended. Review of monthly summaries to analyze data provided in order to gauge progress towards stated goals, identify issues and concerns, and problem solve. Review assessments to ensure the information is included in the person’s applicable service plans. Implement the procedure for the admission of people seeking services, ensuring the efficiency and effectiveness of these procedures. As assigned, serve as the point of contact throughout the Intake process between the referral source, prospective person/family to be served, the DDRO/OPWDD, and agency programs or services. Provide data to the appropriate department designee regarding the Intake systems and processes. Facilitate tours of Agency programs as requested by the prospective person to be served and his/her family. Complete all Intake forms and ensure distribution of completed Intake packets for requested services. Work in collaboration with Program Managers through the referral process and startup of new services for each person. Provide persons and families with education to better understand the intake process and service options and choices. Present all available Agency services and assist prospective persons to be served and his/her family during the entire Intake process. Act as an advocate for prospective persons to be served in obtaining services requested within the Agency. Maintain reasonable flexibility in schedule to accommodate people served. Ensure each person’s information is kept up to date and assessments are completed as required in the Agency Management Platforms, such as Therap, and OPWDD Platforms, such as CHOICES. Complete and ensure the person’s DDP2 is current and accurate for each service. Verifies that all DDPs are in alignment with abilities skills, needs, and safeguards. Enters DDP 1 and 2’s into

platform. Assist in the identification and development of new services and supports that provide quality of life choices. Assists support departments such as nursing and Behavior Supports with tracking, including identification of concerns and facilitating meetings as necessary and required by regulation. (Example, use of PRN’s, falls, general problem solving.) Be an expert in and provide training and support in use of Therap. Ability to multi-task and switch between multiple funding sources throughout the day, ensuring regulatory requirements are met when authoring plans and providing services. Participate and observe the provision of services Provides Direct Support Services as assigned. Physical Plant and Safety Ensure the site is a safe environment and immediately address areas of concern. Practice standard precautions and other appropriate infection control practices. Maintain knowledge of fire evacuation plan and ensure plan is followed. Report and document potential and actual safety hazards Contacts with Others Develop and maintain positive relationships with internal and external stakeholders and demonstrates high diplomacy. This includes the person, their family, Care Coordinators/Care Managers, and external service providers. Maintain effective working relationships with all service providers, community resources, and natural support networks. Ensure that internal and external written and verbal communications regarding the agency are positive and reflect our mission, vision, and values, and respect the person’s right to privacy and confidentiality. Ensure concerns are addressed promptly and professionally. Demonstrate respect for diversity and inclusion Meet with individuals, family members and members of a person’s circle of support routinely through intake meetings and person-centered planning to develop an effective plan for service for them. Additional Functions Participate in agency committees, initiatives, projects, and trainings. Participate in the development of policies and procedures for program operation. Perform direct support responsibilities, as assigned. Write newsletter articles. Serve as an agency training instructor. May be required to use private vehicle to go to meetings, provide services, and to transport the people we support. Identify opportunities to use Therap fully and ensures these opportunities are maximized. Work with the Therap team to identify and develop new services and programs to utilize Therap. Assist program associates with using Therap Represent programs and other departments at Life Plan meetings as needed.

KNOWLEDGE, SKILLS, AND ABILITIES

Errors may be difficult to detect and could have a fiscal impact and impact on the health and well-being of the people served and the agency. The person in this position can establish effective relationships and work in a team environment to meet team and agency goals. The person in this position can exercise discretion in handling confidential information. This includes medical, financial, and personal information of associates and people served. The person in this position has basic Microsoft Office skills The person in this position has basic Microsoft Excel skills, including the ability to enter and sort data and produce graphs. The person in this position demonstrates knowledge of introductory accounting and computer skills. The person in this position has good written and verbal communication skills and counseling and crisis management skills and leadership. The person in this position must maintain a working telephone.

QUALIFICATIONS
Required:

A human services professional with at least a bachelor’s degree in a human services field (including, but not limited to; sociology, special education, rehabilitation counseling, and psychology). Has at least one year of experience working directly with persons with intellectual disability or other developmental disabilities. And is qualified under one of the following categories: An individual who holds at least a bachelor’s degree in a professional category listed below (additional requirements may apply) An Occupational Therapist or Occupational Therapist Assistant A Physical Therapist or Physical Therapist Assistant A Psychologist with at least a master’s degree A Social Worker (Graduate or bachelor’s degree) A Speech-language pathologist or audiologist A professional recreation staff member with a bachelor’s degree in recreation or in a specialty area such as art, dance, music or physical education. A professional dietitian A Doctor of Medicine or Osteopathy. A registered nurse. CPR/First Aid/AED and SCIP Certification within the first six months of hire (training provided by the agency) Alignment with agency’s mission, vision and code of ethics. Demonstrate respectfulness, professionalism, effective teamwork and empathy such as active listening.

ADDITIONAL QUALIFICATIONS/REQUIREMENTS

Where position requires driving: Valid driver’s license for the state in which the person resides. Acceptable safe driving record. Where position requires driving individuals served by the

Motor vehicle with up-to-date inspection, that is registered and insured. Will be required to enroll in LENS (Licensed Event Notification System).

AUTHORITY AND ACCOUNTABILITY
People:

This position provides guidance and coaching to Direct Support Professionals in all departments of the Community Supports.

Operations:

This position has responsibility for writing plans of service and teaching associates to implement those plans. This position will also observe or demonstrate service delivery. This position is required to review service documentation and review and approve monthly summaries. Operates with minimal supervision.

Budget:

This position has minimal budget responsibilities and must adhere to expense guidelines and programs. Errors can result in significant cost to the Agency.
