The Coordinor will function in support of caregivers, Veterans and family members throughout the Medical Center and in the community and will support the development and implemention of General Caregiver services. This includes working with the facility CSP Program Manager to provide program support and oversight of the PCAFC under the umbrella of Caregiver Support Program services. The task may include supervising

CSP PCAFC

staff, tracking consults and workload, assisting, covering…. conducting frequent needs assessments to implement educion, wellness, and support groups. In addition, the Coordinor must be a skilled communicor capable of developing effective partnerships with local, ste and federal agencies as well as non-governmental organizions. The Caregiver Support Program Coordinor develops and implements the PCAFC specific to his/her area of assignment and insures compliance and collaborion with local facility and the VA Central Office Caregiver Support Program initiives in concert with the facility CSP Program Manager. The Coordinor administers informion and analytical systems to evalue and enhance the quality of service provided to caregivers, families and Veterans. Develops and implements informion systems to track service activities including s, referrals, caregiver demographics, equipment utilizion and caregiver and Veteran sisfaction. Develops implements and evalues clinical guidelines and protocols to establish approprie utilizion of services.

Program coordinors the GS-12 grade level are administrively responsible for a clinical program providing trement to Veterans in a major specialty area, such as but not limited to: Spinal Cord Injury, Homeless Continuum Veteran Program, Hospice and Palliive Care Program, Suicide Prevention Program, Veterans Justice Outreach, Caregiver Support Program and Community Nursing Home Program. The program coordinor may be the sole practitioner in this specialty the facility and typically provide direct pient care services in the program area. The program coordinor oversees the daily operion of the program, develop policies and procedures for program operion, and prepare reports and stistics for facility, VISN, and nional use. They may be responsible for the program’s budget, developing and monitoring staff compliance with practice, standards and guidelines on documention, workload, da entry, ethical practice and service delivery. The program coordinor provides analysis and evaluion of clinical program da and computerized programs to identify system-wide trends and needs to enhance the quality of service. They may be responsible for or contribute to the program’s resource and fiscal management, monitoring control points developing the annual budget, opering within th budget, and accounting for appropried funds. The program coordinor is administrively responsible for the clinical programming and prepares reports and stistics for facility, VISN, and nional use. They provide leadership, direction, oriention, coaching, in-service training, staff development, and continuing educion programs for assigned staff.
