Job Description
Team Leader/Supervisor Maryhaven Yaphank, NY The Team Leader provides overall supervision, training and support to staff working in classrooms with individuals who have an intellectual developmental disability.
Essential Functions:
Serves as QIDP (Qualified Intellectual Disabilities Professional) of the Interdisciplinary Treatment Team to ensure quality of all aspects of the program curriculum. Oversees three to four (3-4) classrooms to assure the individuals are engaging in meaningful, productive activities and are receiving the oversight as stated in each individual’s Day Hab Plan. Active in hiring process, training, and overall supervision of all classroom staff to include performance appraisals, corrective actions, commendations and time sheets. Acts as primary liaison to outside agencies, including overview of ITT meetings and administers all written and verbal communication. Ensures adequate staffing levels for daily operation of the Day Hab program. Assists with Activities Coordinator responsibilities in absence of the Activities Coordinators/peers. In absence of Program Manager, a Team Leader may be asked to assist with building responsibilities. Assist with program re-certification process, through

quality assurance reviews. Ensures written documentation is of high professional quality and is forwarded to outside agencies in a timely manner. Provides feedback to policy and procedure drafts. Assists with development and maintenance of policies and procedures. Maintains confidentiality. Participates in emergency procedure including assuring coverage at assigned residential facilities when there is an emergency closure of the Day Habilitation Program. Maintains supplies to assure programming can be carried out, replenishes when necessary, and is conscious of the budget while ordering supplies. Drives agency vehicles as needed for community outings, etc. Keeps monthly progress notes and provides comprehensive quarterly summary of each individual’s progress while working on their valued outcomes as well as daily data on the aforementioned valued outcomes. Attends all departmental meetings/in-services as required and assigned. Well versed in usage of Precision Care and monthly paperwork process.
Education and Qualifications:
BA in Human Services field required with one year experience in the field, and 1 year experience working in a supervisory capacity overseeing staff for administrative and clinical direction. Maintains valid NYS driver’s license within agency criteria.
