NURSE PROGRAM SPECIALIST SHARED WELLNESS 185
Titus Avenue, Warrington, PA 18976 General Description Shared Wellness is founded on the belief that people living with intellectual and developmental disabilities are not defined by their diagnosis and, with the proper support, are able to achieve their dreams and become valued members of their communities. A Nurse Program Specialist with Shared Wellness will be responsible for managing the program needs of a specific caseload and the nursing needs of individuals across the organization, as determined by the Director of Nursing. They will create a collaborative and efficient workplace with attention to agency quality standards and policies. Role and Responsibilities Program Duties Manage support and services ensuring health and safety for all assigned individuals. Caseload subject to change Supervision of DSPs for assigned program individuals.
Note:
supervision of all nurses will be the responsibility of the Director of Nursing Schedule the individual’s medical appointments and oversee all medical care Ensure the individual’s residence is safe, homelike, and reflects the individual’s preferences Manage and supervise staff around Shared Support South policies and procedures, as well as program needs Ensure reporting and assessments are kept up to date, timely, and meet requirements (including but not limited to: Program Books, Quarterly reports, Annual Assessments & Lifetime Medical History, ISP, BSP, MARS, daily notes, monthly paperwork) Facilitate the development of a team focused on supporting the individual Provide training and modeling of support strategies, as needed to ensure successful implementation Assign team roles to staff to ensure operations for the care of the individual Understand appropriate legal and regulatory requirements for 6400 regulations Facilitate monthly circle meetings for the individual and their support team Develop and implement services in accordance with the individuals ISP Attend weekly staff meetings at SSS Office and host regular meetings with your DSP/Nurse teams Facilitate and support individual relationships and communications with their social circle Facilitate, troubleshoot, and support program activities/issues that may arise All other duties as assigned Direct and Indirect Nursing Duties Work direct nursing shifts each week. The number of shifts will vary based on need. For skilled nursing shifts worked beyond your required 40 hours, you will receive crisis pay. Assist Director of Nursing in completing nursing assessments for individuals in residential programs Assist in coordinating nursing needs for assigned individuals Complete health screening for specific individual(s) based on a specified criteria with support from Director of Nursing Collaborate with agency wide departments and professionals to streamline operations as they relate to nursing needs of the individual Collaborate with outside agency professionals (medical specialists) to ensure the highest level of medical care is being provided to the individual you support. This is especially important when an individual with complex medical needs is hospitalized and we need to maintain communication Participate in on call nursing rotation coordinated by Director of Nursing Support on-call manager with nursing related needs when Director of Nursing is out on PTO You will be supported by the Director of Nursing when you are out on PTO Coordinate with Director of Nursing and contracted medical professionals to carry out recommendations related to individuals health and psychiatric needs. This may require scheduling appointments and facilitating follow-up with Program Managers All other duties as assigned Required Skills In depth knowledge of ID, and dual diagnosis Proficient in G-Tube and J-Tube use and care Proficient in medication administration Ventilator and Tracheotomy experience Knowledge of legal regulations and best practices in healthcare Willingness and ability to keep abreast of changing standards in nursing administration Excellent ability to lead and develop personnel Clear and concise communication skills Outstanding organizational and time-management skills Aptitude for problem-solving, critical thinking, and prioritization Excellent Documentation skills Compassionate, caring, and punctual Qualifications and Education Requirements Current nursing license (LPN) in state of PA Associate’s degree in nursing Current CPR and first aid Minimum 2 years of direct nursing experience Minimum 1 year Trach, Vent, and G Tube experience Reliable and able to meet deadlines with minimal prompting Person-centered thinking experience or training Creative problem-solver Shared Wellness is an equal opportunity employer.
