POSITION SUMMARY

The Qualified Intellectual Disabilities Professional (QIDP) develops and implements the Individual Program Plan (IPP) in accordance with applicable state and federal regulations. The QIDP integrates, coordinates, and monitors the individuals’ IPP and provides training of staff in collaboration with the clinical, medical, and operational personnel and provides oversight for the progress of individuals on their goals and the general implementation of the

IPP. DUTIES AND RESPONSIBILITIES

Advocate for the individual through program development and collaboration with all service providers, ensuring that there is consistency among all services received, and any discrepancies or conflicts between programmatic, medical, dietary, social, and vocational aspects of the individual’s assessment and program are resolved
Lead the Interdisciplinary Team (IDT) meetings and participate as a member of the IDT to ensure appropriate, aggressive, and consistent Active Treatment for individual residents in the Intermediate Care Facilities for Individuals with Intellectual Disabilities (ICF/IID)
Ensure follow up to IDT meeting recommendations for services, equipment, or programs through activities including but not limited to outpatient therapy sessions, community integration, recreational activities, and everyday living tasks.
Use and update the electronic health record as needed to keep the information in the record current for assigned individuals supported
In accordance with established regulatory and Elwyn policy and protocol, assist the Director of Admissions with the admissions process by establishing the initial plan and initiating the individual’s orientation to their new program, and coordinate discharge planning with individual’s team at time of discharge to ensure smooth transition out of the program
For new admissions, implement the Admission Support Plan, by assisting individual in adjusting to new living arrangement, providing baseline information on activities of daily living for the completion of the skills assessment, and creating the initial IPP with the team within thirty (30) days of admission and annually thereafter
Observe staff implementation of identified goals with the individuals, review data and progress, and revise IPPs based on individual need and progress
Ensure all team members have the correct, up-to-date information regarding the individual’s needs and program changes to prepare staff to speak with family, day services, or supports coordination organizations when needed
Maintain a safe, healthy, and stimulating environment and ensure quality of clothing to meet life needs, quality life choices and social and recreational opportunities
Coordinate all services being received by the individual, including but not limited to out patient therapy sessions, community integration, recreational activities, and everyday living tasks to manage psycho-social aspects of daily life
Monitor and provide ongoing quality improvement functions as part of Adult IDD Services’ Continuous Quality Improvement activities such as record auditing, data auditing, active treatment auditing, etc.
In conjunction with Program Supervisor, organize and maintain individual record books
Provide staff with in-service training on a proactive and consistent basis including hands on training as needed
Maintain and exceed, when possible, ICF/IID federal standards in programming for annual review
Implement all procedures regarding individual’s budgets including but not limited to oversight of monthly finance reports and oversight of individual’s spending money in collaboration with the Finance Coordinator and Benefits Coordinator
Assist with the coordination of and attend regular nursing meetings and clinical rounds to ensure compliance with regulatory requirements for medical appointments
Assist with operational responsibilities as needed to include petty cash submission of receipts and any necessary forms
Maintain effective communication between management, staff, families, and day program for the benefit of individuals supported
Actively carry out building safety routines and find ways to teach individuals supported of same, take the initiative to correct safety hazards such as appropriate use of Personal Protective Equipment (PPE), sanitation and hygiene, and/or report hazards to appropriate supervisory personnel
Actively participate, taking leadership role when assigned, in meetings and in-services, clinical rounds, quality improvement activities, and any training
Attend training and in-service as scheduled
Complete incident reports per Elwyn policy including but not limited to the review of the incident and necessary follow-up
Perform cardiopulmonary resuscitation (CPR) and crisis intervention using agency-trained protocols
Perform other duties as assigned

EDUCATION/EXPERIENCE/SKILLS REQUIREMENTS

Meets or exceeds the following education and experience levels
One (1) year of experience working directly with persons with intellectual disabilities or other developmental disabilities AND
Possesses a bachelor’s degree in a Human Services field
Ability to work independently
Ability to work in a fast-paced environment and manage and prioritize multiple, often competing, priorities
Demonstrated ability to work effectively as part of a team and to lead teams
Demonstrated strong attention to detail
Demonstrated strong time management and organizational skills
Demonstrated excellent judgment with the ability to independently solve problems and make decisions with little or no need for direct supervision
Demonstrated ability to anticipate future needed changes or identify problem areas and take effective actions
Must possess excellent customer interaction, collaboration, presentation, and written and verbal communication skills
Demonstrated basic experience with Microsoft Office applications, including Word, Excel, and Outlook; PowerPoint
Experience using/knowledge of Electronic Health Record (EHR) preferred
Must be able to perform CPR, lifting and transferring, and crisis intervention techniques using agency-trained protocols
