Having an understanding and demonstrating knowledge of these components is necessary. The Program Specialist will work independently to complete required documentation in a timely manner. In addition, they must also possess leadership skills to guide Direct Support Professionals in completion of daily documentation, goal setting and recording and community inclusion tasks in accordance with Everyday Lives and Person-Centered Practices. Qualifications College degree with 2 years working with IDD/Autism population OR An associate’s degree with 4 years working with the IDD/Autism population Residential or Community-based programming experience Key Job Responsibilities Coordinating and completing high-quality assessments Responsible for participating in the individual plan process, development, team reviews and implementation of the plan Ensuring the individuals on your caseload are receiving all services as outlined and authorized in their service plan Complete monthly and quarterly paperwork as necessary by program requirements in a timely manner Organize and maintain individual files in accordance with program requirements Assist and monitor that benefits remain active and in good standing. Apply for SNAP, EBT, and other offered benefits as necessary with individuals input and assistance Establish a rapport with individuals while creating a person-centered environment Provide guidance and leadership to direct care staff and housing tenancy staff as needed Coordinate health care appointments: ensure follow-up appointments are scheduled and documented accordingly. Coordinate teaching and skill development for individuals and team members as deemed necessary Create a healthy, engaging and positive home environment for Aegis individuals Input goals, notes and authorizations into TherAp for new admissions and those on your caseload. Update as needed and in conjunction ISP changes. Ensure identified services, goals, and outcomes are accurately documented in TherAp and as written in the individual’s plan Onsite visits to homes on your caseload on a monthly basis Participate in weekly supervision with your direct supervisor Participate on assigned committees as assigned and when needed Work with Aegis leadership on projects or assignments to grow the Aegis brand and company Complete and maintain annual trainings Other duties as assigned and in conjunction with changes in 6100, 6400, and 6500 regulations Knowledge, Skills & Abilities required: Knowledge of TherAp useful, but not mandatory Strong written and verbal communication skills Demonstrate a strong understanding of 6100 regulations Ability to implement Aegis mission, vision and values Ability to work independently and complete projects, assignments and tasks on time Possess a valid driver’s license and reliable transportation Travel throughout Lehigh Valley expected up to 30% of working hours Physical/Mental Demands The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
