Senior Coordinator – Case Management 3.1 3.1 out of 5 stars 927 E Lynnwood St, Allentown, PA 18103 Senior Coordinator-Case Management This is a remote position, but some travel is required . Visits to locations in the area are required on a regular basis. Mileage reimbursement is provided for all necessary travel. Qualifications A program specialist shall have one of the following groups of qualifications: 1. A master’s degree or above from an accredited college or university and 1 year of work experience working directly with individuals with an intellectual disability or autism. 2. A bachelor’s degree from an accredited college or university and 2 years of work experience working directly with individuals with an intellectual disability or autism. 3. An associate’s degree or 60 credit hours from an accredited college or university and 4 years of work experience working directly with individuals with an intellectual disability or autism. Essential Functions The Case Manager Supervisor is responsible for the overall quality and implementation of resident service plans as dictated by Keystone Community Resources and residential regulatory requirements. Develops Agency forms, procedures, and policies related to regulatory/funding agency requirements as it relates to Case Management responsibilities. Responsible for training and supporting Program Specialists and Case Managers in Case Management/Program Specialist duties. Directly supervises assigned Case Managers. Designated as contact person for Licensing/Monitoring visits. Responsible for regular meetings with Program Specialists/Case Managers. Responsible for random auditing of charts. Maintains a caseload of up to 10 clients. Develops Individual Service Plans with client, family, funding agency representative, and members of treatment team for assigned caseload. Facilitation of Individual Service Plan/Individual Treatment Plan meetings and Interdisciplinary Team Meetings, in cooperation with the Funding agency representative; and completion of necessary documentation including but not limited to progress/status summaries, progress notes, and monthly/quarterly updates for assigned caseload. Serves as a primary contact for families, agencies, and other professionals involved in clients’ care for assigned caseload. Maintains regular contact with client for assigned caseload. Coordinates services among providers to ensure continuity of care for assigned caseload. Attends professional meetings as needed to represent the client and Keystone Community Resources. Coordinates admission and discharge process with Program Coordinator and Residential Manager. Communicates regularly with Residential Manager/Coordinator to ensure implementation of treatment goals in the residence; regularly updates Manager on any changes for assigned caseload. Provides necessary coverage in homes, in emergency situations, to ensure safety and welfare of clients. Assume on-call coverage as assigned. Participates in required in-service training for continuing professional development. Other duties may be delegated to the Senior Coordinator as necessary, fair, and appropriate under the supervision of the Director of Case Management. #PRIOAM
