Description •Please apply if you are between the ages of 21 and 27•We are currently seeking a Youth Supporter Partner to join our team. High Fidelity Wraparound (HFW) is a team-based collaboration, they help families identify their strengths and community resources. Under the direction of the High-Fidelity Wraparound Supervisor or the designee, the High-Fidelity Wraparound Youth Support Partner (YSP) is a young adult who has successfully navigated system involvement. This provides an empathetic link to the youth in wraparound through which the Y.S.P. supports and extends the work of the H.F.W. facilitator to ensure the phases, activities, principles, and theory of change are accomplished by providing direct services to the youth to better ensure an individualized approach. Youth Support Partner:
Credentialing Process:

Complete all requirements within 12 months to become a fully credentialed youth support partner.

Support Role:

Provide information about Pinebrook Family Services and the High-Fidelity Wraparound (HFW) process to assist youth in engaging with their individualized plans.

Resource Provision:

Offer personal and professional life experiences and appropriate resources like support groups, family activities, and recognition events.

Advocacy:

Advocate for the “youth voice” in program operations and development, ensuring access, voice, choice, and involvement.

Team Participation:

Maintain active participation in the team, providing excellent communication about the family’s story, culture, strengths, and needs.

Documentation:

Ensure timely and professional documentation and keep the supervisor informed of any issues. WHY you will LOVE working with us:

Life Experience:

 Candidates must have personal life experience with systems such as mental health, adoption/foster care, homelessness, or probation before the age of 19. Current recipients of High-Fidelity Wraparound Services are not eligible.

Age and Sensitivity:

Applicants should be between 21 to 27 years old and demonstrate sensitivity to cultural and socio-economic differences.

Home Visits:

The role requires visiting families’ homes, with mileage reimbursement provided.

System Knowledge:

Candidates must understand and have experience with various systems, including schools, mental health, child welfare, juvenile justice, and healthcare.

Community Resources:

Extensive knowledge of community resources and the ability to access them is essential.

Personality:
