We are looking for someone to help change the lives of families residing in Charleston. The ideal candidate is self-motivated and can help bridge the gap between community resources and family needs. Engaging community partners in building stronger relationships to foster economic growth and development among our families is necessary. Helping families unlock their potential to reach self-sufficiency is crucial and guiding them through the process is a requirement. The primary purpose of this position is to plan, coordinate and organize various programs related to resident self-sufficiency. The position assists residents in becoming self-sufficient and economically independent by engaging with and counseling residents to determine the community’s needs and providing useful resources and services. In addition, the incumbent is responsible for maintaining positive working relationships with local social service and provider organizations. This position is subject to a three-year funded grant. Illustrative Examples of Work Coordinates the delivery of services to program participants (including working with internal staff and external service providers as needed). Maintains records by submitting written reports detailing experiences with participants to superiors, along with recommendations. Develops and maintains a comprehensive profile of treatment services and resources Provides motivation to participating individuals and families in order to assist them to reach agreed upon goals. Maintains a substantial client caseload; including maintaining related files, records, and documents. Communicates with local agencies in providing services and resources for program participants. Conducts periodic assessments of program participants.

PROGRAM ESSENTIAL DUTIES

Provides support and guidance to public housing residents/participants in order to assist them in achieving their educational goals. Participates in the recruitment of program participants; distributing flyers in the community, conducting informational sessions, following up on referrals and developing partnerships with local partners. Answers inquiries (face to face, telephone and email) from general public and local partners relating to program requirements, application status and disbursements. Monitors and evaluates participant and program progress; obtains transcripts, soliciting feedback from participants and local partners. Provides social work case management and navigation services designed to assist clients and their families obtain stability. Provides onsite case management services in the area of employment linkage. Encourages and promotes an environment that promotes self-empowerment to assist clients in meeting their individual goals.

CONTRACT & PROGRAM MANAGEMENT ESSENTIAL DUTIES

Prepares case-related reports including program participant outcomes, Maintains complete and accurate documentation of service objectives and outcomes as well as other services in accordance with federal guidelines.

MINIMUM TRAINING AND EXPERIENCE

Minimum requirements include a bachelor’s degree in Sociology, Social Welfare, Psychology, Community Organizing, Health Care, Drug Abuse Rehabilitation, or other related behavioral discipline. Two years of related work experience also required. An equivalent combination of education and experience may be considered. Valid Driver’s License required. Knowledge of the basic principles, methods, and techniques of education, training, employment, health care, substance abuse, social welfare, elderly/disabled services, and other support program administration and organization.

MINIMUM QUALIFICATIONS OR STANDARDS REQUIRED TO PERFORM ESSENTIAL JOB FUNCTIONS

-Must be physically able to operate a variety of machines and equipment including a computer, vehicle, telephone, and calculator. Must be able to exert up to five pounds of force occasionally and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects. Sedentary work involves sitting most of the time, but may involve walking or standing for periods of time. Must be able to lift and/or carry weights of five to ten pounds. -Requires the ability to apply logical thinking to define problems, collect data, establish facts and draw valid conclusion. -Requires the ability to record and deliver information, to explain procedures, to follow and give verbal and written instructions, to counsel and teach employees and clients. Must be able to communicate effectively and efficiently with persons of a variety of educational and cultural backgrounds, and use counseling and marketing terminology.
