Key Responsibilities:
Referral Management:
Process and respond to referrals for the Prevention of Blindness Program.
Eligibility Determination:
Determine eligibility for assigned Prevention of Blindness cases, maintaining contact with the applicant throughout the eligibility determination process.
Service Plan Development:
Develop individualized service plans for consumers once eligibility is confirmed.
Medical Service Coordination:
Coordinate medical services for all eligible consumers, ensuring treatment is completed and follow-up care is provided.
Ongoing Communication:
Maintain communication with doctors’ offices and consumers to ensure timely and effective treatment delivery.
Case Closure:
Ensure all cases are closed appropriately and according to agency policies.
Community Outreach & Education:
Conduct presentations and consultations to community and agency partners regarding the Prevention of Blindness Program. Collaborate with community partners to raise awareness about eye disease prevention and available services.

Minimum and Additional Requirements
A bachelor’s degree and professional experience in the ophthalmic field or similar medical field. An associate degree and two (2) years of experience in the ophthalmic field or similar medical field or a high school diploma and four (4) years of experience in the ophthalmic field or similar medical field may be substituted for the required bachelor’s degree.
