Minimum Qualifications:
High school graduate or GED equivalent with some work experience preferred. Must be a person who has a general knowledge of
HIV/AIDS
issues. Some computer experience and valid driver’s license required.
Preferred Qualifications:
Associate or Bachelor’s degree in Human Services or related field. Must have knowledge of HIV/AIDS and other public health issues. Experienced with Microsoft Office Suite and database software.
Summary:
The Outreach Specialist will assist the Care Coordination Team to provide linkage to medical care for persons living with HIV that are newly diagnosed or are not in care. The Outreach Specialist works a flexible schedule that includes evenings and weekends.
Responsibilities :
- Identify, recruit and retain eligible and appropriate individuals for education, health and prevention services.
- Assist care coordination to provide access to medical and other services for clients.
- Actively participate in developing and facilitating a recruitment strategy for case finding, testing and care engagement.
- Facilitate home visits, client transport and other client contact as assigned.
- Ensure documentation of client services.
- Maintain outreach database.
- Participate in intake process for clients recruited for the program.
- Make referrals to resources and programs as directed by the Linkage Coordinator.
- Develop a rapport with consumers who are newly diagnosed or those out of care.
- Provide peer counseling, consumer education and community education.
- Complete in-service training and continuing education as required or requested.
- Attend meetings and training as requested or required.
- Actively participate in developing and facilitating a marketing and outreach plan with the Care Coordination Director.
- Assist with reporting and evaluation.
- Other duties as assigned.
Supervisory Relationship:
Reports directly to the Care Coordination Director.
