Develops, provides, and evaluates educational programs to meet the identified needs of the population(s) served including patients, families, staff, and the community. Monitors and ensures accuracy of documentation on patient records. Maintains all required reports, records, and statistics related to program. Monitors the quality of care delivered to the population(s) served on a continual basis; identifies quality issues and integrates process improvement (P.I.) into practice.
