Performs duties as required to support the elimination of community-based violence.
EXAMPLE OF DUTIES
Develop a comprehensive network of resources that addresses the needs of the population who are victims or perpetrators of community-based violence.
Develops pre- and post-test questionnaires and other instruments to measure the effectiveness of case management services.
Prepares analytical and interpretative reports on case management.
Manage a caseload of residents who are referred to MPHD for Behavioral Health and WellnessCoordinate and evaluate services that identify approaches to eliminate gaps in services and resources.
Review and integrate instructional methods, materials, in in-service provision to 1manage a diverse caseload of participants.
Participate in the dissemination of program information through attending conferences, seminars, and meetings to promote, publicize, and gain support for the programs.
Keeps abreast of current developments and trends in the provision of case management services.
Explore and/or place clients in specialized programs as needed.
SUPPLEMENTAL INFORMATION
Certified Case Management (CCM) designationBachelor’s degree in social work, psychology, and/or nursing. Knowledge of the principles, methods, and techniques of one or more behavioral, social, or related sciences.
Experience working with the public.
Bilingual speaking is a plusAbility to conduct interviews, assess needs, and develop an appropriate course of action.
Ability to research and analyze complex data and write reports.
Ability to communicate effectively, both orally and in writing.
Ability to establish and maintain effective working relationships internally and externally.
