Description Job Summary:

The Maternity Home Grant Visit Educator (MCGVE) will be responsible for training clients on the maternal home visitation model and conducting outreach program activities for participants and their families in identified Hamilton County, Tennessee. The goal is to improve health outcomes and provide comprehensive support to families in need. Responsibilities include training clients on the home visitation model, ensuring they understand the goals, procedures, and benefits, conducting outreach activities to engage participants and their families, including information sessions, community events, and one-on-one meetings. Identify and connect with potential program participants in Hamilton County providing ongoing support and follow-up to ensure program participants are benefiting from the services offered, collaborating with community organizations, healthcare providers, and other stakeholders to promote the home visitation model and expand program reach. Maintain accurate records of client interactions, training sessions, and outreach activities, monitoring and evaluating the effectiveness of the outreach program, making necessary adjustments to improve outcomes staying informed about best practices and advancements in the field of home visitation and community health. The MHGVE generates reports from Erlanger’s E-chart system, EPIC, to pull down Maternity Home Visitation Grant (MHVG) quality metrics and interprets quality metric data to identify potential opportunities for improvement. There is also an emphasis on presentation, education, and explanation of data and home visitation standards. The MHGVE must express a deep understanding and knowledge of MHVG quality metrics and reports when explaining reports to MHVG nursing staff and providers. Provides training when needed for Home Visitation nursing staff on how to accurately capture patient quality data for MHVG metrics through face-to-face workshops and email/phone communications. Serves as subject matter expert on home visitation standards, updates, and requirements. Maintains files, records, policies and procedures as required by home visitation national center to meet home visitation requirements. The MCGVE reports to the Maternity Home Visit Clinical Resource Specialist which reports directly to the Perinatal Grant manager or Director of Women’s Services. This position is involved in a team-based approach to care. Team members are trained to meet the highest level of function for their role as per the State of Tennessee guidelines, is trained and assigned (as per their role and responsibilities) to coordinate care for patients and is trained and assigned to support patients/families/caregivers. Participates in the practice’s quality improvement process and performance evaluation.

The Maternity Home Visit Grant Program Educator will work with the Clinical Resource Specialist to ensure the overall running of the home visitation program. Assisting and coordinating meetings, parent graduations and gifts, data entry, data analysis, and record keeping for grant funding. Weekly monitoring of data entry and reviewing needed documentation by home visitor nurses. The educator will review all metrics to ensure quality is being met based on the national home visitation metrics. Will work in conjunction with Clinical Resource Specialist to promote program to all stakeholders, set up and help with advisory board meetings, and performs any other duties as directed.

Primary Responsibilities:

Project Coordination Provide logistical support to the maternity home visitation team including meeting preparation, data collection and analysis, distribution of materials, visitation gifts and items Communicate with the MGVG external partners and other stakeholders as needed Conduct periodic assessment of MHVG projects and support care teams in their implementation; track success with MHVG projects Works collaboratively with all MHVG team members to achieve MHVG goals Identify projects that require sustained tracking for external success Reporting requirements Run all data for existing projects on a regular basis, develop creative methodologies for data dissemination; prepare and distribute monthly/quarterly summary reports to care teams and other stakeholders where needed Work with MHVG Clinical Resource Specialist to prepare and complete external reporting requirements for MHVG and select Quality Contracts Track, document, and submit quarterly grant education to the Clinical Resource Specialist Provide and monitor data, data analysis, and statistics to the home maternity national office when asked to complete MHVG Implementation Provide support to MHVG Clinical Resource Specialist and others in MHVG team in the day-to-day coordination of activities for MHVG implementation Support Team-Based Care and other similar meetings/ trainings Compile information, maintain detailed knowledge of each initiative, and document progress toward achieving MHVG goals Assess implementation progress; supply qualitative and quantitative feedback to teams and individual referral providers Prepare training materials and other learning tools for distribution to teams as supported by national agency Provide one on one or group coaching to staff and care teams to support information dissemination and changes related to MHVG practice transformation Prepare background information, develop presentations, and represent MHVG leadership at Women’s and other standing meetings as needed Work with a diverse team of staff at all levels
