The Permanency Specialist will be an integral part of the Child and Family Team for DCS custodial children in full guardianship or those who have a dual goal of adoption or permanent guardianship
Job Specifications Essential Functions:
- The Permanency Specialist will be an integral part of the Child and Family Team for DCS custodial children in full guardianship or who have a dual goal of adoption or permanent guardianship. The Permanency Specialist will participate in CFTMs for these children.
- The Permanency Specialist will adhere to all DCS/Harmony policies and procedures as well as the Adoption Best Practices Manual and Adoption & Subsidized Permanent Guardianship Procedure Manual.
- The Permanency Specialist will participate in CFTMs for children entering full guardianship to ensure appropriate permanency tasks are captured on the child’s permanency plan. The Permanency Specialist may also be required to consult with the child’s team as requested.
- The Permanency Specialist will work with the Family Service Worker when the permanency goal is changed to a dual or sole goal of adoption or permanent guardianship to discuss options with the foster family should they decide to proceed with providing permanency for a child/youth placed in their home.
- The Permanency Specialist shall document all permanency discussions and ongoing permanency efforts into TFACTS by the 15th day of the month.
- Once a child enters full guardianship, the Permanency Specialist will be responsible for the following: o Ongoing efforts to achieve permanency o Suitability of the foster/adoptive home placement o Continued commitment of the foster/adoptive family o The child’s adjustment to the foster/adoptive family o The foster/adoptive family’s adjustment to the child o Foster/adoptive placement stability o Safety of the child in the foster/adoptive home environment • Permanency Specialist will monitor current services and supports provided to the foster/adoptive family and any additional support services, assess if additional supports are needed to maintain the placement.
- Permanency Specialist will have a minimum of one phone call per month with the FSW to discuss progress of the case and any needs identified.
- The Permanency Specialist will conduct regular face to face contact with youth.
- The Permanency Specialist will meet face to face with any new child assigned to their caseload within 10 business days.
- The Permanency Specialist will make initial contact with the family and/or private provider within 3 business days.
- The Permanency Specialist will actively engage in developmentally appropriate life book discussions and activities with the child or youth.
- The Permanency Specialist will complete and update Pre-placement and Presentation summaries.
- The Permanency Specialist will refer pre-adoptive/SPG families to Adoption and Guardianship Preparation Training.
- The Permanency Specialist will complete full disclosure and presentation books.
- The Permanency Specialist will complete an Individual Recruitment Plan within 30 calendar days of entering full guardianship.
- The Permanency Specialist will initiate an archeological dig within 90 calendar days of a child entering full guardianship.
- The Permanency Specialist will register children on AdoptUSKids within 60 calendar days of entering full guardianship and maintain their registration.
- The Permanency Specialist will respond to any and all inquiries from any recruitment source within seven days.
- The Permanency Specialist will partner with private providers to respond to inquiries from recruitment sources.
- The Permanency Specialist will refer children to recruitment events and ensure they attend any needed events such as Heart Gallery, In My Own Words, or other events as deemed appropriate.
- The Permanency Specialist will schedule and facilitate quarterly recruitment meetings for all children in full guardianship, initiate and complete monthly child and family searches, and document the outcomes.
- The Permanency Specialist will gather and prepare home studies for selection CFTMs. The Permanency Specialist will schedule these selection meetings.
- The Permanency Specialist will attend permanency reviews as needed or requested by leadership.
- The Permanency Specialist will maintain the full guardianship spreadsheet relative to the permanency status of each child on their caseload.
- The Permanency Specialist will complete specific tasks for finalization including, but not limited to: o The Permanency Specialist will complete the subsidy work for adoption cases o The Permanency Specialist will obtain the Final Decree of Adoption o The Permanency Specialist will prepare the DCS case record for sealing
- The Permanency Specialist will participate in Harmony’s Independent Living Camps.
- Other job duties as negotiated to meet the needs of the agency.
Job Specifications:
- Bachelor’s degree in social services, education, humanities, or related field.
- A minimum of five years of experience in the field of adoption and/or foster care.
- Experience in training facilitation and family counseling required.
- Strong leadership skills which promote productive team dynamics, including schedule coordination, problem solving, conflict resolution/mediation, and maintaining goal-directed activities.
- Strong organizational skills and ability to work independently as well as with a team.
- Proficient use of computer programs (including electronic records, Word, and Excel) and familiarity with navigation of the internet to document case notes and other activities in a timely manner.
- Excellent communication skills (interpersonal, written, and oral).
- Capacity to create rapport with child and all members of the Child and Family Team.
Other:
- A valid driver’s license, automobile insurance, and reliable transportation is required. A significant amount of travel is required including, but not limited to, travel to client or foster/adoptive family homes, attendance at CFTMs or other meetings and travel to Harmony main office and Camp Montvale for supervision, team meetings, and/or staff meetings.
- Ability to lift minimum 10-15 pounds required.
- Must clear all Harmony required pre-employment background checks. No person will be employed who has been convicted of any offense against another person.
- Participates in Harmony staff meetings and trainings to include, but not limited to completion of mandatory trainings to support compliance with COA standards and professional best practices. Trainings and meetings may include overnight.
- Ability to contribute to discussion in team meetings or other meetings.
- This position is home office based; candidate must reside in the Nashville or surrounding area.
Requires a minimum 40-hour work week from Sunday to Saturday each week.
