- Performance Specialist, you will play a pivotal role in driving the planning, execution, and evaluation of population health initiatives and performance improvement projects across a dynamic healthcare network.
You will lead efforts to enhance clinical and financial outcomes, engage stakeholders, and ensure seamless communication across teams, practices, and leadership. Essential Duties & Responsibilities Population Health Initiative Management Direct multiple population health initiatives/projects (e.g., clinical initiatives like AWV-TCM) within a healthcare network. Educate and assist in problem-solving to promote the implementation of population health and clinical initiatives. Support the preparation and delivery of presentations related to population health and clinical initiatives. Ensure proper training on population health and its role in improving continuity of care for patients. Conduct routine meetings with staff (including PCPs), documenting discussions, action plans, and feedback. Data Analytics & Reporting Possess general knowledge of data analytics and reporting requirements. Review data to identify trends and opportunities for revenue growth and clinical improvement. Apply analytical tools to research trends, expenditures, and utilization of healthcare providers/practices. Collect, compile, and analyze data to measure and report provider and program performance. Design and implement dashboards to track and manage assigned project performance. Program Management & Strategic Planning Lead planning, evaluation, rollout, and monitoring of the Performance Enhancement Series (PES) program across the ACO. Develop and maintain customized strategic action plans for practices, tracking deliverables and timelines. Conduct quarterly performance assessments using financial, quality, and utilization data to identify underperforming practices for PES enrollment. Oversee the implementation of Additional Network Participation Standard Agreements for practices with persistent performance or financial challenges. Monitor compliance with benchmarks (e.g., Low Panel and Billing Days policies) and escalate practices not meeting targets. Stakeholder Engagement & Communication Coordinate PES communications, including letters, consultant briefings, and performance reports. Liaise with provider consultants and the Medical Director to support follow-ups, reassessments, and practice transitions.
- Work effectively across all levels of the organization, maintaining clear communication and managing expectations with leadership.
Physician Advisor Program Oversight Manage scheduling, coordination, and tracking of Physician Advisor (PA) meetings across the ACO. Collaborate with the Medical Director to support PA onboarding and align group meeting agendas with ACO priorities. Performance Monitoring & Contingency Planning Proactively manage shifts in project scope, identify potential crises, and devise contingency plans. Conduct project post-mortems to create recommendations for improvement. Monitor progress of PES practices using tools like Caretracker and the Super DocACO App, escalating noncompliance or roadblocks as needed. Maintain accurate documentation, dashboards, and trackers for internal review and auditing purposes. Provide monthly summary reports to leadership on PA engagement, compliance findings, and unresolved issues. Education & Training Educate physicians and providers on population health initiatives, including the PES program’s role in improving patient care. Assist in the creation and distribution of training materials and educational resources. Additional Responsibilities Design and implement cross-functional initiatives to optimize processes and improve efficiency. Support company philosophies, objectives, decisions, and policies. Perform other duties as assigned within the scope of responsibilities. Ensure adherence to HIPAA, confidentiality, and organizational privacy standards. Key Performance Indicators (KPIs)
Data, Reporting & Accountability Dashboard Accuracy:
Maintain ≥ 98% accuracy in dashboards, including variance analysis and narrative summaries.
Timely Reporting:
Ensure 100% on-time submission of monthly performance reports by agreed timelines.
Issue Resolution:
Resolve urgent data issues within ≤ 5 business days of identification.
Population Health Initiative Management Initiative Completion:
Achieve ≥ 95% on-time completion rate for assigned population health projects (e.g., AWV-TCM).
Training Delivery:
Provide population health training to 100% of relevant staff within assigned timelines.
Educational Resource Accuracy:
Maintain ≥ 98% accuracy in training materials and presentations.
Communication, Collaboration & Leadership Meeting Documentation:
Document 100% of PES meetings and action logs within 48 hours, with full distribution to stakeholders.
Leadership Satisfaction:
Achieve ≥ 90% satisfaction score from department leaders on clarity and effectiveness of communications, measured semi-annually.
Operational Support & Performance Optimization Performance Assessments:
Complete quarterly performance assessments using financial, quality, and utilization data to identify underperforming practices for PES enrollment.
Action Plans:
Develop and maintain strategic action plans for 100% of enrolled PES practices within 30 days of assignment.
Additional Network Participation Standard Agreements:
Implement contingency agreements for 100% of identified underperforming practices within established deadlines.
Stakeholder Engagement & Department Readiness Engagement Metrics:
Ensure ≥ 90% of planned consultant and practice engagements are documented and logged within the reporting period.
Compliance and Professionalism HIPAA Compliance:
Maintain 100% adherence to HIPAA and organizational privacy standards.
Competencies:
To perform the job successfully, an individual should demonstrate the following competencies: Analytical and advanced problem-solving strategies. Knowledge of and experience collaborating with Provider Communities. Experience with Electronic Medical Record (EMRs) or Health Information Management (HIMs) systems. An elevated level of engagement and emotional intelligence Progressive operational experience within a medical center, clinical group, or hospital setting. Experience:
Required:
Bachelor’s degree in business administration, healthcare administration or related field.
Preferred:
Master’s degree in a related area. Three+ years of experience in healthcare project management, clinical group operations, or hospital performance improvement.
