Pay rate: $28.00/hr Temp 12 Months (End date no later than June 2026, depending on the start date.) Number of Weekly Hours – 30 hrs per week (Will need some flexibility) Work Schedule/Days to work – Primarily Monday through Friday, 8:15 to 5 pm. Page 1 of 2 OVERVIEW The Local Efforts towards Addressing Disparities in DeKalb (LEAD DeKalb) Project team requests a contractor to serve as the community engagement project planner, assisting with coalition building, community health improvement (CHIP) action planning, and supporting project sustainability. The overall role is to help coordinate the efforts to work collaboratively with the Live Healthy DeKalb (LHD) Coalition community members, organizations, and stakeholders to address issues affecting the community’s well-being as outlined in the LEAD DeKalb workplan and the Community Health Improvement Plan. The Live Healthy DeKalb Coalition (LHD) is a group of residents and representatives from nonprofits, healthcare providers, and government agencies who work together for a healthier DeKalb. LEAD DeKalb works alongside LHD to address the burdens of chronic disease prevention by promoting healthy eating and active living in Black/ African American and Hispanic/Latino communities in DeKalb County. The Community Health Improvement Plan (CHIP) efforts are to assess and prioritize health needs and disparities, analyze the needs and disparities, and develop an action plan for the priority areas and community engagement. This project is part of the Local Efforts towards Addressing Disparities in DeKalb (LEAD DeKalb) initiative, which aims to improve health, prevent chronic disease, and reduce health disparities in DeKalb County. The LEAD DeKalb initiative is funded by the Centers for Disease Control and Prevention (CDC) as part of their Racial and Ethnic Approaches to Community Health (REACH) program.
SCOPE OF WORK
With support from internal LEAD DeKalb program activities, the expected duties and responsibilities of the contractor are to assist with: A. Community engagement and improvement action planning
- Documenting the documentation process for the Community Health Improvement Plan (CHIP) Create standardized templates and tools for tracking action plan implementation progress
- Establish performance metrics and evaluation methods to assess impact Document evidence-based strategies and best practices to be incorporated Generate regular status reports on implementation progress
- Identify potential barriers to implementation and develop contingency plans
- Ensure implementation strategies align with available resources and community needs SOW – Community Engagement Project Planner Page 2 of 2 Create communication materials to explain the implementation process to stakeholders Design feedback mechanisms to allow for continuous improvement B. - Evaluate LHD Coalition’s existing partnerships, governance, and engagement strategies.
Expand membership & engagement by recruiting diverse organizations (i.e., faith- based organizations, businesses, schools, and grassroots groups). Work with coalition members to set clear goals and identify where and how the LEAD DeKalb priorities align with the coalition’s goals and actions teams. Help integrate LEAD DeKalb efforts into local policies and programs for long-term sustainability. Improve communication and collaboration by identifying frameworks to enhance engagement. Collaborate with the CHIP Steering Committee to engage coalition members in the development and execution of action plans outlined in the CHIP. C. Grant Development & Sustainability Identify federal, state, and local grant opportunities that align with LEAD DeKalb priorities. Assist in writing proposals for foundation grants and federal funding sources. Identify/establish opportunities for long-term funding partnerships.

Grant Support and Sustainability o Identify grant opportunities and a roadmap for applying. o Prepare at least one funding application for LEAD DeKalb sustainability. o Stakeholder (e.g., interested parties) engagement plan o Monthly status reports and updates on coalition development and funding efforts
