Post-Care Project Coordinator 3.0 3.0 out of 5 stars Hickory, NC 28658 JOB Catawba County Social Services is seeking a Post-Care Project Coordinator to lead the coordination, quality assurance, and replication of the county’s nationally recognized Success Coach Service. This program supports children and families exiting foster care by building protective factors and reducing risks. The Post-Care Project Coordinator is responsible for overseeing replication support, maintaining program fidelity, managing data analytics and grant deliverables, and supporting program growth.
This position plays a vital role in expanding the reach of the Success Coach Service and ensuring its effective, trauma-informed implementation across partner agencies and statewide post-permanency programs.
EXAMPLE OF DUTIES
Project Management & Grant OversightManage deliverables, outcomes, and outputs for multiple grants and projectsComplete quarterly (or as required) grant reports in coordination with the Post-Care SupervisorDevelop and maintain grant budgets; ensure ongoing compliance and communication with grantors and stakeholdersProvide regular updates to leadership and organize project-related meetingsCollaborate with external partners to establish a certification program for the Success Coach modelReplication & Technical AssistanceTrain and support public and private agencies replicating the Success Coach modelDevelop and lead monthly technical assistance calls with replication sitesCoordinate ongoing site-specific training and maintain fidelity to contractual and program standardsPartner with NCDHHS to support statewide replication through the Post Permanency Support Services contractFidelity Monitoring & Continuous Quality ImprovementConduct monthly case reviews and track fidelity metricsMaintain quality assurance tools and practice profilesAnalyze data and recommend improvements to enhance program efficiency and impactDevelop data-informed reports to share results with funders, stakeholders, and leadership
