Full Job Description Position Summary:

The Downtown Rising Team Lead is tasked with leading a team dedicated to engaging individuals experiencing homelessness in various public settings, including gas stations, streets, parks, and other outdoor locations. This pivotal role centers on building trust and relationships with a highly vulnerable population, providing immediate support, and linking individuals to essential services. The Team Lead coordinates with broader community resources and facilitates the transition from life on the streets to stable housing and essential supportive services, thereby reducing the prevalence of homelessness and improving individual well-being in the community.

Essential Duties and Responsibilities:
Leadership and Organizational Support:

Lead, motivate, and manage the street outreach team, ensuring alignment with HOPE Atlanta’s mission and vision. Collaborate with program management to organize team meetings and contribute to program implementation discussions and community events. Provide ongoing training and serve as a resource for team programs and standard operating procedure (SOP) training.

Client Engagement and Management:

Direct and enhance the team’s efforts to engage with and build trust among homeless individuals, with a focus on diverse and vulnerable groups. Oversee the assessment of individuals for service eligibility, intake processes, and the development of Individual Service Plans (ISPs).

Service Coordination and Referrals:

Ensure effective connections for clients with relevant services and benefits, including healthcare, housing, and emergency services. Guide the team in assisting clients with essential documentation and support through the housing application process.

Documentation, Reporting, and Compliance:

Rigorously maintain and update client files and documentation within the Client Track HMIS system. Produce timely reports on outreach activities and outcomes.

Collaboration and Community Engagement:

Foster partnerships and collaborations to prioritize outreach efforts and integrate clients into the Coordinated Entry system. Lead the team in community engagement activities, including the annual Atlanta CoC Point in Time count. Attend relevant CoC meetings, participate in case conferencing, and provide insights to improve program processes and performance.

Additional Duties and Responsibilities:

Adapt to additional duties as required, focusing on client welfare and service delivery excellence. Ensure team readiness to provide services beyond conventional hours, as necessary.

Knowledge, Skills, and Abilities:
Advanced Knowledge:

Leadership Skills:

Exceptional communication, crisis intervention, documentation, organizational, and problem-solving skills.

Technical Skills:

Proficiency in MS Office Suite, Pathways, and HMIS database systems. Experience with Salesforce Voucher entry.

Abilities:

Strong adaptability, interpersonal sensitivity, teamwork, initiative, and cultural awareness.

Qualifications:

Bachelor’s degree in social work, psychology, public health, or a related field with at least one year of experience in a leadership role working with the homeless population, or an equivalent combination of education and experience. Demonstrated experience in conducting street-based outreach and rapid rehousing. Proven ability to manage teams in dynamic environments. Valid Georgia driver’s license with a good driving record and reliable transportation.
