The Student Fellowship Program Health Worker will provide culturally appropriate individual and group health education, coaching, informal counseling, case management, care coordination, and advocacy in order to promote, facilitate and improve access to health services, academic and career planning for Lynn youth. The Student Fellowship Program Community Health Worker works with the student fellowship program team, the school based health center team, Lynn Public schools, and other community agencies, as appropriate to promote, maintain, and improve the student participant’s health, academic and career planning. The primary responsibility of the Student Fellowship Program Community Health Worker is to educate, advocate and coach Lynn youth who require additional support to meaningfully address medical, social, and psychological needs. The Student Fellowship Program Community Health Worker collaborates closely with the student fellowship program staff, Lynn Public School staff, nurse practitioner and behavioral health provider, and other members of the school based health center team. The Student Fellowship Program Community Health Worker works with the student participants and/or family at LCHC, in the school, school based clinic, or other Lynn Community Health Center sites, and community settings, as well as the participant’s home. Additionally, the Student Fellowship Program Community Health Worker will participate in LCHC and Lynn Public Schools community outreach efforts, as appropriate and as assigned. This may include participating in health screenings, health education programs or counseling at health fairs and other similar community events. The Student Fellowship Program Community Health Worker also plays an important role in the community on behalf of the student participants and/or families to provide assistance with skill building, facilitating linkages with other community based and natural support systems, and advocating for community needs. The Student Fellowship Program Community Health Worker reports to the School Based Health Center Site Manager or his/her designee with secondary support and oversight by grant program manager
QUALIFICATIONS
Bachelor’s degree in health sciences, social work, sociology, human services or related field preferred Ability to work with and relate to individuals and families from diverse backgrounds and experiences Strong knowledge of local community, social service, and cultural resources Strong organizational, interpersonal, attention to detail, time management, multi-tasking, and problem solving skills History of good attendance and positive work attitude Excellent communication skills and ability to set personal boundaries Ability to establish positive and supportive relationships with and among participants and colleagues Ability to work independently and collaborate with a team Ability to motivate others and thrive in a fast paced environment Willingness to openly address and acknowledge issues of addiction, substance use and mental illness Ability to develop, adapt, and execute outreach plans Excellent English written and verbal skills Computer literacy and ability to document appropriately in electronic health record Must have a valid driver’s license and transportation Bilingual and multilingual candidates encouraged to apply
EXPERIENCE AND TRAINING
Three or more years experience as a community health worker or case manager in a community health setting; and/or related experience in health/human services, social services or advocacy. Experience in outreach within mental health, community-based treatments or medical comorbidity. Ability to openly address and acknowledge issues of addiction, substance use and mental illness. Training and or certification as a community health worker preferred. Community health worker may obtain certification within one year of Massachusett’s Department of Health certification approval and/or or within six months of hire, whichever comes first. Experience providing health education and knowledge of health promotion/wellness preferred. Attendance at School Based Health Center Community Health Worker training as determined by the Massachusetts Department of Public Health
SPECIAL REQUIREMENTS
The essential duties of this position present risk of exposure to airborne infection, body fluids and blood-borne pathogen. Annual blood borne pathogen (BBP) Training is required. Up to date immunization and annual TB testing is required. Hepatitis B vaccine and annual flu immunization are strongly recommended. All necessary vaccines are available at no charge at the health center. A Declination form must be signed if the Hepatitis B or other recommended vaccines are declined. Results of inquiry to Criminal Offender Record Investigation (CORI) must be acceptable under health center and Lynn Public School standards.

Treat all participants and/or families in a welcoming and professional manner. Demonstrate professionalism by appropriate attire, attendance, attitude, and behavior within work and community settings. Contribute to the team effort by supporting all team members and maintaining an open and positive attitude. Welcome new employees to the team by assisting with their orientation to the team and mentoring them, as requested.
HOURS OF WORK
Monday through Friday school hours. Occasional evening and weekend hours may be required.
DUTIES AND RESPONSIBILITIES
Work with the student participant and/or family and the student fellowship program staff to identify, develop and implement a cultural competent and participant directed/centered individualized treatment/care plan. Conduct outreach and engagement activities that support student support services, career and academic planning, continuity of care, including re-engaging participants and/or families in care if they miss appointments and/or do not follow up on treatment or need supportive services. Conduct an initial and periodic needs assessments, including assessing barriers and assets (e.g. transportation, community barriers, social supports); student participant and family or caregiver preferences; language, literacy and cultural preferences. Promote student participant and/or family treatment adherence through assessing student/family readiness to make changes, assisting students in making changes in daily routines, identifying barriers and assisting participants with developing strategies to address barriers. Collaborate with other providers, staff, Lynn Public Schools and community resources to enhance coordination, quality and efficiency in care. Provide culturally competent support, individual and group education, guidance, mentoring, and coaching regarding chronic disease management, comprehensive preventive care and self-management goals. Organize and or facilitate health education groups that promote wellness. Organize and or facilitate digital literacy trainings that promote career and academic success. Assist student participants and/or families with effectively navigating health care and social service systems, including arranging for transportation. Support participants and/or families in preparing questions for provider’s visit and assist participants in understanding care plans and instructions, and tailoring communications to appropriate health literacy level. Support participants and/or families in preparing questions for academic and career development and barrier reduction. Provide support for hypertension self-management to individuals and their families. Show initiative in working creatively with participants and/or families to address their identified needs and help them move towards successful resolution of challenges. Provide culturally competent care coordination and support services for participants and families depending on individual needs and preferences. These include but are not limited to: Link participants/families with resources and empower them to use them to their advantage advocate for individual and community health needs for the participant/family direct practical assistance, i.e. filling out forms, making appointments as appropriate Network/collaborate with community leaders, area health providers, voluntary organizations, and other who can contribute to ongoing assessment of the health needs of the target population.
Conduct community outreach to:
provide information/make referrals to/coordinate needed services identify community resources that provide needed support for participants and/or families provide health education to individuals and groups on prevention and public health risks and problems educate staff/providers about target population and community characteristics and needs Participate in meetings with supervisor and attend team and staff meetings at the health center and Lynn Public Schools. Participate in staff development trainings and external educational offerings to maintain and improve knowledge base. Accurately document all participant and/or family interactions in a timely fashion in the designated electronic health record. Documentation includes but not limited to: care plan progress, telephone calls, home visits, time with providers, case management, travel time etc. Keep logs, data on participant contacts, as required by funding source.
CUSTOMER SERVICE
Demonstrates an understanding of customer service principles by successfully completing on-line training. Utilizes the principles of customer service when interacting with participants/clients, team members and staff from other departments. Appropriately handles or seeks support when customer service breakdowns occur.
CULTURAL COMPETENCE
Demonstrates awareness of culture by successfully completing the on-line training on Cultural Competence. Utilizes an appreciation of and respect for diversity when interacting with participants, team members and staff from other departments. Responds appropriately or seeks support when confronted with cultural biases or conflicts.
EMERGENCY SITUATIONS
Is aware that in order to respond promptly and appropriately to emergencies, any staff member may be assigned duties, which differ from those in the job description or may be assigned temporarily to a different location. Understands that all staff should have a Personal Emergency Plan in place to best respond to his/her job responsibilities should health center emergency arise.
