We’re looking for the right person to lead our team of Community Health Workers (CHWs) at MaineGeneral Health — someone who’s ready to make a real impact in the lives of high-risk, low-income patients.

MaineGeneral Health runs an innovative, evidence-based healthcare program that’s proven to improve health outcomes for our most vulnerable populations. At the heart of this model are CHWs — trusted community members who provide structured advocacy, social support, and care navigation to help patients reach their health goals. CHWs are “natural helpers” — people who share lived experiences with the patients they serve, including background, language, and socioeconomic status. We’re proud of our CHW team, and now we’re looking for a passionate leader to support, guide, and grow their work.

Key Responsibilities:

Lead, support, and coach a team of CHWs serving patients in 6-month cycles Track and analyze performance data to ensure strong patient outcomes Collaborate with clinical care teams and community organizations Facilitate team meetings and educational opportunities patient support groups Manage project budgets and basic accounting for program expenses Maintain program compliance, productivity, and participant satisfaction Oversee data collection and reporting for grant-funded projects

The Ideal Candidate Has:

Leadership experience in a non-profit or community-based setting A background in clinical social work or case management (preferred) Deep knowledge of healthcare, social services, and public benefits systems Familiarity with resources and organizations in Central Maine Strong skills in using data to monitor performance and guide decision-making Experience with grant writing and management (preferred) Excellent communication skills, both written and verbal A college degree (MSW or Mental Health and Human Services MPH preferred) A passion for supporting low-income individuals and families If you’re ready to lead a mission-driven team and help shape better health outcomes in your community, we’d love to hear from you!
