As our ACT Team Qualified Professional, you will be responsible for providing evidenced-based practices, including assisting your clients in creating their overall treatment plan, monitoring needs, linking to necessary resources, establishing crisis plans, and identifying barriers treatment. This team covers Wake County, NC – we provide mileage reimbursement! Pay Starting pay is $22.00 per hour. Schedule Monday – Friday, 830a-430p. You will be eligible for medical benefits the first of the month following your first 30 days. We have options for a standard plan, as well as an HSA with a $300 initial contribution and up to a $25 per paycheck match. You would receive 10 paid holidays as well as PTO hours which would accrue at a rate of 10 hours per month. After 6 months you would be eligible to enroll in our 401(k) plan which has a 4% match. We also provide company paid life insurance, short- and long-term disability, in house and paid training opportunities, and paid credentialing. SouthLight is also an approved employer for the Public Service Loan Forgiveness Program (PSLF). Job Responsibilities of an ACT Team Qualified Professional (QP) Provide specialized training in housing and tenancy supports Facilitate training of independent living skills, establish access to community services and agencies, assist individuals in establishing support networks in the community and assess individual needs on a daily basis Coordinate services with the program, staff, inpatient facilities, and detoxification facilities and establish linkage to residential programs, self-help programs and community resources Review treatment progress and monitor and provide continuous consultation with other Assertive Community Treatment team sand community agencies to ensure the consistent therapeutic management Qualifications of an ACT Team Qualified Professional (QP) Education and Experience QP status 4 year degree in Human Services + 2 years’ experience with MH/SU population 4 year degree non-Human Services + 4 years’ experience with MH/SU population Knowledge, Skills, and Abilities Ability to easily connect with people with profound communication skills A constructive approach towards members Ability to maintain confidentiality of the information collected from the members and their families Ability to tackle complicated issues with ease Considerable knowledge of the dynamics and problems of addiction and current practices of treatment Strong commitment to the rights and abilities of each person with a severe and persistent mental illness to live in normal community residences About SouthLight Healthcare Tap into your passion at SouthLight Healthcare.
