Shift:

Monday-Friday Conducts monthly visits to each Family Care Home to ensure compliance with certification and regulatory requirements. Assists in re-certification process of all providers with Family Care Coordinator from WSDC. Assists Director with scheduling and tracking all training and inservices for Family Care’s Providers. Develops and monitors implementation of Residential Habilitation Plan. Performs Home Liaison role as described in Family Care Manual. On-call responsibilities.

ESSENTIAL FUNCTIONS /RESPONSIBILITIES

Supervises and oversees all activities of assigned Family Care Providers. Conducts at least monthly visits to each Family Care Home to ensure compliance with certification and regulatory requirements, completing Family Care Home Checklist and other component activities required by Family Care policy on Home Visits. Conducts at least one annual unannounced visit for the homes on their caseload. Provides overall guidance and support for assigned Family Care Providers. Maintains and updates a record for each Family Care Provider and each person served including purging based on agency records retention manual. Attends Life Plan planning meetings and develops a Residential Habilitation Plan to assist each person in achieving their valued outcomes. Attends applicable meetings when necessary for persons served to ensure the highest quality of care is provided. Monitors implementation of Residential Habilitation Plan by Family Care Providers. Responsible for completing monthly narrative summaries of Residential Habilitation Plan. Prepares billing for assigned caseload and submits for approval to Director. Provides direct assistance to people served in accessing needed medical, dental and clinical or other services and supports. Work collaboratively with the Family Care, RN to ensure continuum of care through out each family care residence. Responsible for completing initial and maintaining SNAP and Medicaid recertifications. Responds to critical and emergency situations that may arise within the Family Care Program, including after hours situations. Reviews fire evacuation plans and drills at monthly site visits and conducts an observed drill once yearly. Ensures that monthly fire drills are conducted. Safely transports individuals to and from activities and appointments in emergency situations or when the FCP is unable to do so due to unusual circumstances. Complies with all agency policies and OPWDD and procedures. Other duties as assigned.

MINIMUM QUALIFICATIONS

Associates Degree and 3-5 years of related experience in human services field . NYS Drivers License that meets agency policy. Ability to effectively respond to inquiries or complaints from individuals, staff, regulatory agencies or members of the community. Ability to use mechanical and medical adaptive equipment as utilized by individuals at site, min-van and/or other transportation vehicle. Ability to lift and transfer an individual both manually and mechanically as required by his/her prescribed program/plan. Move, reposition and place individuals in pieces of equipment. Lifting requirement of 50lbs. Physical agility/ability to react to emergency situations. CPR and First Aid certification required.
