We are looking for… A Habilitation Supervisor who can perform a wide range of duties: Responsibilities and Duties Supervises and evaluates program to ensure compliance of all regulatory standards. Implements program policies and procedures as needed and routinely monitors for compliance. Coordinates and participates in provision of services. Coordinates referrals, admissions and discharges of Individuals in cooperation with families, referring agencies, and providers. Reviews and updates plans to address changes in Individuals’ interests, needs and family situations. Ensures that staff develop obtainable individualized goals for Individuals that foster valued roles within their communities. Monitors Individuals satisfaction and assures that changes are made when appropriate and in a timely manner. Explores and oversees the development of new community opportunities that facilitate Individuals involvement. Routinely evaluates service delivery for effectiveness and makes recommendations as needed. Ensures documentation is prepared, maintained and submitted in accordance with regulatory requirements.. Provides regular and ongoing supervision to all staff. Regularly conducts staff meetings to communicate policy and procedure changes, promote team building and a productive work environment.
