out of 5 stars 101 Sedgewood Drive, Rock Hill, SC 29732 Transition Specialists provide quality care and support that promotes positive outcomes for youth and families served in NHTC residential programs and community counterparts. Transition Specialists provide support to our youth clients and their families by utilizing core principles, emerging evidence base and acknowledged best practices using the Building Bridges Initiative (BBI) model during the client’s residential stay and up to 90 days after residential treatment discharge at a minimum to ensure smooth and successful transitions to homes. Provide emphasis on strong collaboration and coordination of services between providers, families, youth, and community advocates to assigned clients through support, collaboration, linkage to community resources, imparting knowledge, enhancing parenting skills and participating in any collateral contacts on behalf of the client in order to achieve its goals. Responsible for maintaining documentation to ensure accountability of ongoing progress and/or regression of all assigned clients and families.

PRIMARY RESPONSIBILITIES

Provides transitional services as stated in key deliverables to assigned clients and families within recognized ethical and professional standards. Maintains accurate and timely client records as required by NHTC and MCO’s standards. Complies with Performance & Quality outcome measurement data collection, audits and reporting requirements. Maintains appropriate documentation and submits service notes and other required paperwork on a weekly basis and according to standards of the program.. Prompt attendance is required for required family education, staff meetings and other agency training. Ensures proper training and implementation of the BBI practice model adopted by NHTC. Participates in discussions of the on-going support of families in order to provide input and direction with the supervisor and other staff working with the children and families. Travels to school sites/homes or other community locations. Abides by all NHTC policies and procedures. Maintains all professional licensures, certifications, and required training held at time of hire as applicable.

PROFESSIONAL CHARACTERISTICS

Communicates with the supervisor any training, development, disciplinary, and program issues in a timely manner. Ability to engage parents, guardians, and other community partners effectively. Skills/experience in facilitating program assessments and utilization of BBI standards including Motivational Interviewing skills. Ability to convey clearly a non-judgmental, open minded, culturally aware/sensitive demeanor. Coachable and open to constructive criticism. Active listening and summarizing skills. Guide parents, guardians to solutions.

INTERAGENCY RELATIONSHIPS

Professionally represents the Agency in the community especially around relevant issues impacting children and families with mental health diagnosis. Attends outside Agency meetings as needed i.e. CFT Meetings, Family Home Visitations, DSS and mental health reviews when necessary, and maintain strong relationships and attend meetings with NH Case Managers, Therapists and relevant staff, and other community based service providers. Bachelor’s Degree in relevant field and at least one year of experience Must live in local area (Mecklenberg county)
