Community Support Program:

Provide case management and support services to adults residing in Chittenden County who are living with psychiatric disabilities and some with co-occurring substance use challenges. This position offers a combination of administrative responsibilities and direct service. Great opportunity to support individuals in a community-based setting.

Responsibilities:

Determine quality interventions, maximizing resources and fostering the clients’ independence Deliver case management services in the least restrictive community environment respecting clients’ rights to self-determination Complete administrative and clinical reports

Requirements:

A combination of education and relevant experience Valid driver’s License and use of a personal vehicle

Structure:

Full time (37.5 hours) Non-Exempt AFSCME Union (dues will apply) Hourly rate starting at $23.22 We offer a competitive salary commensurate to experience
