JOB SUMMARY :
The Clinical Housing Navigatorposition is responsible for acting as the liaison for BHS®’ residents with the Housing Specialists and external case management and providing crisis management services for residents of BHS®. This position will aid in the success of each resident by adding a clinical perspective to our safe and affordable housing model of service. This position will also coordinate home visits, referrals and connections for outsourced case management and to identify opportunities for resident housing stability, recovery, and reintegration. The Clinical Housing Navigator will also aid in closing the gap of services rendered by community mental health providing agencies (Case Managers) and residents housed. This role is also responsible for working closely with the Housing Specialists in minimizing attrition from the program and filling all vacancies by conducting intake assessments. This role will also review the status of residents with local community partners for development of life skills, sustaining success and wellness and to connect residents with community resources in meeting/achieving stabilization, recovery and resident service needs in an effort for tenants’ self-sufficiency and reintegration back into society becoming contributing members of our community.
ESSENTIAL FUNCTIONS / JOB DUTIES
Take ownership of the holistic needs and comprehensive assessments and case management services to all residents including intake assessment, benefit assessment, progress monitoring, individual money management, tenant education, advocacy and referrals, and assists in engaging residents and families in the development and execution of service implementation plans with their Case Managers. - Manage referrals and waitlist for Broward Housing Solutions’ (BHS) Scattered Site Housing Units.
- Receive and review all applications referred by community partners and Broward County’s Coordinated Entry for BHS’ Rental Subsidy Program.
- Verify income, assets, and review all other items for completeness and accuracy to ensure compliance with program requirements and funding regulations.
- Assess applicant’s housing histories and barriers including credit history, rental history and prior evictions, criminal history, registered sex offender status, outstanding debts, outstanding warrants.
- Coordinate with the Housing Supervisor, Property/Maintenance Manager, and the CPO to create action steps to manage current and potential vacancies and reduce overall fiscal impact of attrition including: Determine make-ready dates and unit turnover time frames Track move-outs Schedule move-ins
- Maintain high occupancy rates across BHS’ Rental Subsidy Program Ensure referrals are being processed promptly and efficiently Elevate issues (unit status, applicant status, etc.) that may prevent occupancy Client Resource Coordination
- Using the Housing First model, utilize Motivational Interviewing, Harm Reduction and Trauma Informed Care to help clients improve their quality of life and increase connection to stable permanent housing opportunities.
- Maintain a comprehensive understanding of available community resources and program eligibility criteria to ensure clients are adequately informed.
- Provide linkage, coordination with, and referrals to comprehensive case management services as necessary and during crisis or other incidents of instability
- Facilitate system navigation and advocate on behalf of clients to ensure timely access to services, interventions are tailored to meet the specific needs of BHS’ Residents, and barriers are being reduced across services and systems of care
- Coordinate with the Housing Supervisor, Housing Specialists and the CPO to identify clients who are ‘high priority’ and/or have a need for enhanced care and intervention services.
- As necessary, conduct in-person post-placement visits to assess client barriers, respond to client crisis/emergencies, mitigate conflicts, facilitate housing retention, and reduce housing instability
- Assist clients with obtaining and maintaining public benefits and entitlements including Medicaid, Public Assistance, SSI as well as assisting with problems related to housing or utilities
- In collaboration with the CPO, develop a comprehensive housing crisis response plan that outlines the community’s available resources, and the action steps the team will take to address potential challenges that may compromise housing stability.
(mental health crisis, substance use relapse, decline of Activities of Daily Living (ADLs), non-payment of rent, financial instability, conflicts with neighbors or landlord, and hoarding/cluttering. Partnership Development
- Cultivate and maintain strong collaborative relationships with landlords and community partners and stakeholders
- Serve as a representative in the community. Promote decreasing housing barriers and financial barriers for clients and their families through community advocacy work and maintaining ongoing engagement with the community, landlords, and other housing/homeless/mental health providers.
- Facilitate and schedule case consultation meetings for BHS® resident situations enlisting support of Community Case Managers and BHS’ Housing Specialists. Administrative Tasks
- Ensure Data Systems (WebAuthor, HMIS) are accurate, contain all current client information, and are updated in a timely manner.
- Maintain clientele, staff and agency confidentiality; including complying with all HIPAA regulations as related to the client/tenants housed under BHS’ programs.
REQUIRED EDUCATION AND EXPERIENCE
Education:
Bachelor’s Degree in Psychology, Social Work, Human Services or related field with at least 2 years of related experience. Years of experience may substitute formal education. Master’s Degree Preferred.

- Must have effective verbal and written communication skills with the ability to establish rapport with both internal and external partners.
- High level computer/technology skills to be able to navigate the agency required databases, documentation, and program procedures.
- Familiarity with HMIS / Service Point is a plus.
SSI/SSDI
Outreach, Access, and Recovery (SOAR) training a plus.
- Knowledge of community resources and counseling/social work practices with high-risk populations.
- Experience working with persons in crisis.
- Must have a valid driver’s license and a reliable source of transportation.
- Must successfully complete Level II background and drug screening.
