ESSENTIAL DUTIES AND RESPONSIBILITIES
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions, including the following: 1. Responsible for Day Habilitation Service Plan development for all individuals attending the program. 2. Maintain confidential records of all consumers who participate in the program in accordance with LifeStream policy. 3. Review monthly progress notes submitted by Personal Advocates for accuracy, content, etc. 4. Completes monthly progress reports when applicable. 5. Accesses clinical services as needed in support of consumer programs and needs. 6. Ensures consumer input into DHSP development, personal interests, and programmatic needs. 7. Ensure consumer training occurs according to established guidelines and appropriate documentation is maintained. 8. Maintain regular contact with program manager to ensure appropriate implementation and progress on consumer DHSP goals and objectives. 9. Maintain contact with DDS, Residences, and Parents/Guardians as needed. 10. Act as a member of the team offering input into all programmatic services, schedules, systems, etc. 11. Make recommendations regarding program quality, consumer choices and provide general input to program manager as needed. 12. Provide direct services/coverage when required and requested by program manager. 13. Makes recommendations to program manager relevant to staff training needs. 14. Attends and actively participates in all
DHSP/ISP
meetings and quarterly meetings. 15. Demonstrates flexibility in meeting program needs.
QUALIFICATIONS AND REQUIREMENTS
Maintenance of an acceptable CORI and fingerprinting Valid Driver’s license Negative drug screen Use of personal vehicle for work purposes Commitment to the philosophy of community inclusion EDUCATION and/or
EXPERIENCE
