Job Summary:

The Population Health Manager will oversee health outcomes and care management for the underserved population with a focus on improving care delivery and reducing costs through a seamless model of access and care. The Population Health Manager will develop, implement, and evaluate comprehensive population health data and plans to ensure patients receive excellent medical care. The Population Health Manager will create programs to provide an accurate clinical picture for each patient, provide clinical expertise, support disease management, assess disease states and utilization management, develop and facilitate care plans, and refer patients to the appropriate level of care. This position manages patient recovery through chronic health conditions with a main objective to improve transitions in care for patients, physicians, family, and the community. The Population Health Manager functions as an integral part of the quality and interdisciplinary teams. Qualifications•

Education:

Bachelor’s degree in Nursing, Public Health, Healthcare Administration, or a related field; Master’s degree in Public Health, preferred;

Licensure/Certification:

Advanced Care Manager (ACM), preferred; Certified Case Manager (CCM), preferred;

Experience:

3 to 7 years of clinical experience in a healthcare setting;

  • 2 years of quality management and risk management experience; Experience in care coordination, case management, or managing patient populations, preferred;
Knowledge, Skills, and Abilities :

Knowledge of targeted interventions to address specific health needs; Able to collect, analyze, and interpret data related to population health, patient demographics, disease prevalence, and utilization of care; Proficient with data collection, analysis and reporting as it relates to clinical, quality, and regulatory issues; Able to develop and implement strategies to improve care coordination and patient engagement; Understands population health management concepts, principles, and practices; Able to use computerized tracking tools; Collaborates with community organizations to address social determinants of health; Knowledge of quality improvement methodologies; Able to work effectively amongst interdisciplinary teams; Familiar with population health tools; Strong analytical skills, organized, and detail-oriented; Strong people management skills with the ability to relationship build; Excellent interpersonal skills; Deep understanding of EMR systems; Commitment to our mission of improving health outcomes in underserved communities.

Pay Range:

$53,827
