HealthFirst Family Care Center, a Certified Great Place to Work and wonderful place to make a difference, is seeking a Population Health Department Manager to join our team of mission-minded healthcare professionals. Reporting to the Director of Nursing, the Population Health Manager is the leader responsible for developing and overseeing population health strategies and ensuring alignment with Value-Based Care (VBC) contracts. This role focuses on achieving population health goals through efficient use of resources and by leading a multidisciplinary team to enhance patient outcomes, optimize performance under VBC contracts, and fulfill Patient-Centered Medical Home (PCMH) requirements.

DUTIES & RESPONSIBILITIES

Develop and implement population health strategies to align with VBC goals and objectives. Oversee the Population Health Program team members to ensure effective execution of population health initiatives. Lead the Population Health Steering Committee to guide strategic planning and implementation of population health initiatives. Act as a liaison with operational leadership to develop an integrated population health model that aligns with organizational goals. Meet regularly with payers to discuss contract performance, negotiate terms, and strengthen partnerships. Ensure compliance with VBC contracts and reporting requirements, including timely updates to payer platforms. Manage vendor partnerships for care management services, including chronic disease management and care management, ensuring alignment with population health goals and organizational objectives. Analyze population health data to identify trends, gaps, and opportunities, adjusting strategies as needed to improve outcomes. Collaborate with clinical leaders, external partners, and payers to strengthen population health efforts and achieve desired results. Coordinate and manage activities related to Patient-Centered Medical Home (PCMH) renewal, ensuring compliance with NCQA standards and submission of necessary documentation. Monitor and report on key performance indicators related to population health, VBC, and PCMH requirements.

Requirements:

Bachelor’s degree in Nursing, Public Health, Healthcare Administration, or a related field; Master’s degree preferred. Minimum of 5 years of experience in population health, care management, or a similar role. Strong leadership skills and experience managing multidisciplinary teams. Knowledge of

VBC, PCMH

standards, quality metrics, and healthcare analytics. Excellent communication, organizational, and project management skills.
