Senior Housing Case Manager 3.6 3.6 out of 5 stars Newark, NJ 07102 Company Overview Integrity House has been a pioneer in providing rehabilitation services for individuals with substance use disorders since 1968. With our CARF accreditation and statewide recognition as a leader in addiction services, we are dedicated to helping individuals reclaim their lives through comprehensive support. Summary The Senior Housing Case Manager assists individuals experiencing homelessness or those at risk of homelessness in accessing and retaining safe, secure, and stable housing. This is a full-time, non-exempt position. The employee is expected to work 35 hours per week. Responsibilities Research and compile a list of affordable housing options and landlords in the community. Provide a comprehensive assessment that addresses life domains such as housing, finances, transportation, legal services, vocational, employment, healthcare and family strengths and needs. Assist members in developing individualized housing support plans to help them achieve their housing goals. Facilitate warm referral to services that support housing access and retention, including but not limited to education, job/skills training, and legal aid services. Assist members in identifying and applying for public benefit and rental assistance programs. Assist members in conducting housing searches and completing housing applications. Help members connect to resources that can assist with housing costs and other housing-related expenses. Assist members with lease renewals and housing certification processes. Assist members in addressing circumstances and/or behaviors that may jeopardize housing. Serve as an advocate on the behalf of the individual by providing support to resolve landlord related disputes and other matters. Document services rendered and member progress in electronic health record. Remain current on Housing First principles, MCO eligibility and prior authorization processes, Housing Supports Program provider requirements, and local housing resources.

Required Education and Experience:

Bachelor’s degree in counseling, social work, or another relevant field. At least four(4) years of experience working directly with individuals experiencing substance use disorders and/or mental health conditions.

Specialized Skills, Abilities and Knowledge:

Understanding of Housing First principles. Knowledge of local, regional, and state housing and social services preferred. Ability to educate and link participants to appropriate care resources in the community. Experience interacting with electronic health records preferred. Excellent organizational skills with the ability to manage multiple priorities. Strong interpersonal and communication skills with the ability to collaborate with team members. Understanding of trauma-informed care preferred. Knowledge of substance use disorder treatment, mental health treatment, recovery, and/or social service programs preferred.
