RESPONSIBILITIES
The Case Manager, along with the treatment team, is responsible for identifying each patient and family’s medical, psychological, social, and financial needs during treatment at Hazel House, and to transition the patient to the most appropriate resources after discharge. The Case Manager is responsible for discovering the multi-faceted challenges that patients and families face and alerting the treatment team to any patient setbacks. The Case Manager is responsible for writing comprehensive discharge plan which is approved by the patient’s therapist and medical team. This discharge plan includes where the patient is going, how the patient will get there and the services the patient will utilize when discharged from Hazel House. The Case Manager will provide resources for patients and families needing help with additional treatment programs, including finding a new church, housing resources, employment/career planning, crisis support, financial aid services, social and support groups, educational help/resources, recreational activities, parental support/resources, and interaction with the criminal justice system if applicable. The Case Manager will create and review the discharge plan with the patient, patient family (if applicable) and treatment team. The Case Manager remains available after discharge to answer any questions or provide new referrals if necessary. The Case Manager will manage the adolescent side caseload progress and reports for completeness and to ensure work is completed within required timelines to include post-discharge appointments, coordination of discharges with other departments at HLC. Coordinate and facilitate medical records requests Inspect and encourage clinical compliance Support Utilization Review coordination Coordinate and manage the scheduling of clinical appointments, psychiatric sessions, case management, wellness programs, pastoral care, and enhancements for all patients. Responsible for ensuring all patients are scheduled for regulatory-required sessions, such as BPSA and psychiatric evaluations, within 24 hours of admission. Scheduling & Systems Coordinator has the following responsibilities and duties: Demonstrate flexibility and adaptability to changes in scheduling priorities and clinical requirements. Address scheduling conflicts, cancellations, and rescheduling needs promptly and professionally. Think critically to identify potential scheduling conflicts and work proactively to address them. Communicate with patients to schedule appointments, provide program details, and address any questions or concerns. Maintain clear and effective communication with staff to relay scheduling information, changes, and compliance updates. Collaborate with staff to address any scheduling-related concerns or challenges and ensure the timely coordination of appointments, including therapy sessions, assessments, and program activities. Collaborate with team leaders and staff to ensure programming aligns with regulatory requirements by conducting audits on patient charts to verify accuracy and compliance. Generate regular reports summarizing audit findings and compliance. Collaborate with leadership to ensure adherence to established protocols and standards. Assist in the training and onboarding of new staff regarding scheduling systems. Assists in administrative task as assigned. Other duties as assigned by the clinical director

MINIMUM REQUIREMENTS
