Schedule:

Monday-Friday, 8:30 AM – 5:00 PM Our established and rapidly growing home care agency is seeking a Home Care Coordinator to join our team at our Savannah, GA office. We are expanding our team due to quick growth and need dedicated professionals to support our mission. Our agency provides a range of services, including medical home health care, non-medical home care, infusion, and pharmacy services, with a focus on supporting individuals who need assistance with daily living or have physical or developmental disabilities. As the Coordinator , you will manage schedules, support administrative functions, and participate in an on-call rotation (1-2 weeks per month) to ensure continuous quality care. Administrative and/ or scheduling experience within a medical or healthcare setting Highly detail-oriented with a positive, reliable, and punctual work ethic Strong organizational, multitasking, and communication skills Ability to manage both patient care and administrative tasks with a customer service focus Tech-savvy, with experience using scheduling and healthcare management software/ EMR systems

Responsibilities:

Coordinate and maintain accurate records of patient care, including care plans, schedules, and medical updates Ensure timely and quality service delivery Create and manage monthly schedules for our caregivers Serve as a liaison between patients, families, and the healthcare team to ensure clear, consistent communication Assist with general office tasks and ensure compliance with attendance and care policies Participate in the on-call rotation to support patient and staff needs outside regular hours Ready to make a difference in your community?
